For all pre-DDO:EU quests.. All named loot in quests has the same drop rate on normal hard and elite.
For post DDO:EU quests, droprates are generally higher on hard and elite. (examples: The Mindsunder, the full "sentinels" series, and perhaps black dragon scales in the new "mired in kobolds" quest.
For Casual difficulty, I haven't run it much ever.. But the generally consensus on the forums is that most if not all drops are reduced by 50% or more.. So not a good way to get named loot.
Quests that have epic difficulty available usually massively increase the chance at getting named items. But there are some exceptions (Chains of Flame and Wiz king come to mind, neither drop named items on epic, probably bugged)
Raidquests do scale fully with diffilculty. Droprate on normal is usually 1/6, hard = 1/5, elite = 1/4, epic = 1/2 or better. Raidquests may never be ran on "casual"
You can pull unique named loot on casual, but the drop rate is, as far as I know, lower. If you are trying to farm for an item, don't try to farm it on casual.
