Not ransack. It's similar to the merchant selling lag. You have your items loaded up and hit sell then they don't disappear right away but slowly one at a time. With the chest it is opened, nothing in the chest for a second then they appear one by one. Doesn't happen a lot but it really sucks when it's THE CHEST you want an item from.
Honestly ? Nothing to grunt about !
That's the least harmful side-effect we can suffer from lags
Errata , I misunderstood OP's issue
this is related to what happened to me few days ago in Slave Lords : not received the ingredients in end chest
and no NRLG item (just PP or scroll or others consommables) in regular chests.

Happened to me in PNemisis. Blue diamond and some PP.
I've been getting 2 items in loot about 20% of time on average.
But in some packs it never happened. While in some others quite often. (tethyamar is one where it happens often, while IIRC it never happened in ravenloft.)
Also if nothing else appears on loot list in couple of seconds after opening chest it will not appear even if waiting full minute (tested).
I thought it was just bad loot roll, but could be lag related.
