I assume that you're having a shield? What sort of weapon are you using? What type of armor are you wearing?

Helm of the Black Dragon: insightful strength +3, slot in constitution +7/8 and dexterity +7/8

Flawless Black Dragon(type you prefer) Armor: Relentless Fury, Draconic Ferocity, Armor Piercing 15/20%, Superior Acid Resistance, Haste Guard, slot in Heavy Fortification

Epic Brawn's Spirit: insightful con +2, slot in resistance +7/8

Seal of House Avithoul: Insightful Dexterity +2, Sneak Attack Bonus +5, Exceptional Sneak Attack Bonus +3, Improved Deception

Guardian's Ring: Sheltering +24, Strength +8, yellow augment slot

Guardian's Glasses: Vitality +40, Seeker 7, slot in either protection +7/8 or max dex bonus +2 or Charisma +7/8

    Sage's Sash: Wizardry X, False Life +35

Epic Gloves of the Claw: +4 damage, +20% incite (doesn't stack), strength +6, enhanced intimidate +5, healing amp +30%

Wall of Wood: +18 Universal Spell Power, Natural Armor +6, Healing Amp +30%, Shield Bashing +20%, +108 Devotion, DR/10 Slashing

    Deadly Necklace of Accuracy

    Boots still unknown

For a paladin you'd obviously want a charisma item in there somewhere, so that'd be up to you as to where to toss it in. These are Epic Hard versions, so if you would like to obtain the Epic Elite versions there's nothing wrong with that. Hope this helps!

Insightful strength is wasted on a paladin with divine might.

In that case, you'd obviously get the Dexterity +8 version and leave an augment slot open to do whatever with, probably slot Charisma in.

Identical heal amp percentages don't stack. You could downgrade to the EN WoW, which has +20% heal amp, but then you lose the higher Devotion etc. My lvl 23 Dwarven Defender is currently using PDK set and EN WoW; I've also got the Eveningstar bear cloak for 20% incite (among other bonuses).

I went with more loot gen than I would typically have done. I had between 3 slots, con 10, str 10, natural armor 10, protection 10, seeker 10 and deadly 10. That took up the belt, neck and glove slots.

For shield I went with EE Bulwark, mostly for the con +3 and slot. I stayed away from tower shields. Between gear and enhancements I think my max dex bonus was around 13 so it was easy to cap out dodge as well. I never did find a weapon that I really liked. Nightmare most times and it worked reasonably. I don't think there are any bastard swords that scream out "tank style weapon" anymore for end game. I went with Cannith crafted boots with true seeing and mobility with spell points and globe in the 2 slots. I left the cloak slot as a variable. Many times I would just swap in the whatever appropriate elemental absorption cloak was needed for a particular area. I had 4 of them all with +10 resist on them so they swapped easily.

At the end I was sitting at 160ish AC in Dreadnaught with ghostly, incorporeal and 13 dodge. If I cared to bother, I was hitting just over 200 AC in Sentinel, but on live right now, it's a very poor destiny even for tanks.

For Boots, Halcyon Boots http://ddowiki.com/page/Item:Halcyon_Boots are pretty handy, also I think Madstone Aegis http://ddowiki.com/page/Item:Madstone_Aegis is the best dps shield although is a tower shield. Black dragon set gives better dps while white dragon set gives better hp and ac. A cloak or necklace with deadly of natural armor/resistance is also a good slot for that atributes.

What's your definition of end game? If you're looking for decent gear to run EH, I have some suggestions off the top of my head.
    1) As I mentioned in another thread, stunning blow is an awesome tool, so I actually use a Cannith Crafted shield with +8 stunning on it, even at level 28 (also with +5 enhancement and other goodies on it). The only other non-weapon or shield with a higher stunning blow bonus is the ring from Rusted Blades, but I've yet to pull the one I need.

    2) Epic Spare Hand belt from house C challenges or Cloak of the Bear from Eveningstar challenges for exceptional combat mastery, plus other goodies.

    3) I still find a concordant opposition greensteel item is really helpful. Torq if you're lucky enough to get one.

    4) Deadly of Accuracy item

    5) Seeker item

    6) +10 strength

    7) Purple Dragon knight gloves for 30% heal amp

    8) +10 Natural armor and Protection items

Interestingly, you can slot quite a number of good random loot gen items.

What is your level split? If you have evasion obviously that make a big difference on the gear that you would choose. What type of weapon do you want to use? Are you strength or dex or even con based? Do you have a decent stunning blow?
