More important than actual numbers for HP is determining if your gear is appropriate for your level as you advance.
If you started off with a constitution score of 14 or higher at character creation, and you have taken at least one toughness feat as you leveled up then you are golden. Taking the toughness feat allows you to use enhancement points to gain additional HP. As a fighter you will be able to get 40 or more HP easily for just a few enhancement points.
For level 5-6 try to get a full abishai set from the Chronoscope raid if possible. The cloak alone gives +3 to your con score.
For levels 7-10 Try to get a +4 con item, there is a nice necklace in the Red Fens pack that has +4 con. Also in this level range if possible get a moderate fortification or heavy fortification item.
For levels 11-16 try to get a greater false life item, or a improved false life item. Also if you still do not have heavy fortification the Necro 4 pack allows you to easily farm a helmet called Minos Legens that is great. It gives +20 HP from toughness and Heavy Fort. Figure at level 11 with a +4 con item, toughness item, and a greater false life item you will have 70 more HP just from those things.
At end game you will want to get a +6 item. The quests in amrath drop belts that have +6 con and GFL on them and are easy to attain.
If you have all the appropriate gear I mentioned here, and a 14 base constitution score then It is not unreasonable to expect that as a fighter you should have over 500 hp at level 20.
