Was your party split up?
I've noticed this before when the party was split. Whenever anyone was at a location that would trigger the voiceover it would hit. So half the people were confused about half the voices, and the other half confused about the other half.

Was soloing actually and have noticed this in restless isles {whilst soloing} on many occasions.
Pretty certain it's specific to the isles and not to do with anything else.
