Mine is the Worst!
Logged in one day after an update was running Von and jumped at some Ooze's. Pulled out my Muck Banes and they had LOST the Bane portion!!!!!!!!!!!!
Imagine the fear I felt when I realized I would not be able to easily destroy them.

Every time I log in... I have to reset my keybindings... oh and there is a blackhole in the bottom center of my gamescreen that seems to cause all my hotbars to collapse in and overlap each other just a bit more every time I log.

I have a few:
First time running Prey in Reader's refuge got stuck between the rocks and couldn't't move lol first time using /stuck
While in the rusty nail running chronoscope, jumped onto a table and got stuck and it looked like I was surfing...had to use /stuck again.
Running Raiding the Giants Vault, an animated armor spawned and glitched leaving me stuck and unable to kill it to get out.
Noticing a trend here? LolMember of Upper Caves of Anthia on Ghallanda.

I ran chains of flames and got the blindness curse, no one talked to the dude for me and the curse never got lifted, they completed and left me there, I double recalled, /deathed, relogged, sent a support ticket, the GMs contacted me straight away, he suggested finding a cleric or a silver flame healer, blind I fumbled through the refuge falling down the canyon, stuck blind unable to do anything for 3 days in DDO finally after my third ticket I got a cure blindness and I /death and it was gone, CURED!
