V-sync not working

I have the occasional horizontal tearing, usually in the city, so I turned v-sync on, which gets rid of it in my other games. But here, it didn't seem to do anything. Tried manually setting the refresh rate in game as well, and turned on triple buffering, but the tearing still exists. My frame rate is 60+, higher or lower depending on if I'm in the city or a dungeon, so thats fine, its just the tearing that is bothering me. Only thread I found in regards to an issue like this was about 3 years old, and the solution to that didn't fix the tearing I'm experiencing. Any ideas whats going on?

First, make sure your graphics driver is not forcing "no v-sync.", I believe it would over-ride the application's (DDO) settings.
