Basically you just list your old name and your new name next to it for your alts

Forstall - Forstall (the only one I got to keep)

Did you offend someone? I've heard of a childish, spiteful person creating toons over here with someone's names just so they lost them! I've wondered if the same happened to me too as I know I offended the same person by telling him what I thought of him and I lost over half my names!
