I bought package to start off with level 4 chharacters, can't figure out how to get it started; in "Create" there is no option for this, just level 1. Any thoughts on this??

Create, then log in the new level 1 character. You will be in a zone with a "trainer" that will level you to 4 as you talk to her, then let you spend your enhancement points as well. Then you get some gear on top of it to use as you start.
