Number crunching concerning attack bonus

Hello,

I have a halfling thrower build. When I throw my dagger while standing very close to my enemies (actually in hand to hand combat), my combat log (and dice) show +14 attack bonus. But I got:
+4 base attack bonus
+6 from DEX 22
+1 size bonus (halfling)
+1 halfling thrown weapon focus (feat)
+1 point blank shot
+1 halfling thrown weapon attack I (enhancement)
+1 returning throwing dagger
It makes +15 but combat log shows only +14. I leveled few minutes ago so now I know for sure that base attack bonus, DEX and +1 dagger work and count so the problem must be in one of the other +1 bonuses. Any idea which one fails to add and why?

Just in case, try logging out then back in. I already had some weird stuff happening after leveling up (although it wasn't related to to-hit).

I tried and experienced it both during solo and group play. When I tested it during solo, I didn't use buffs so unexpected debuffing is not a key to this problem (buffs like Bless or Action Boost work normally).

The problem has existed both before and after leveling and logging out and in (thanks to leveling I know for sure that base attack bonus adds correctly).
