I went to ddo.com main website, clicked on the pre-purchase, chose which one I wanted, and a pop-up window appeared asking for email, credit card, etc.

However, this popup window does not prompt...

When I click on it, it opens a "pop up" window, asking for email, credit card, etc. but no where on that screen does it ask me for my DDO Account !!!!!

How will it know which account to apply it...

Yes, absolutely, without a doubt, you've been "creeping" the difficulty of Normal up, and up, and up until Normal is just too hard for most people at high level.
