Individual quests and quest knowledge is a huge factor - I can solo r4 on a well gear, multi-life toon - in older content. Newer content is jacked way up in difficulty to compensate for the uber...
have you tried relogging? unslotting an augment removes the slot until you relog.
No matter what the devs do (of any game, not just DDO) they're going to lose -

an end game, fully geared tank will have an AC well over 300 (as well as 300+ PRR and MRR)- my pajama wearing level 30 (non-completionist - non-optimized) monk has an AC of 210
FoM also does not protect you from paralysis by drow poison or being webbed by bebilith web attacks
If you're THF, you should probably be putting your level up boosts into STR (not DEX) and if you're soloing you essentially are tanking so you will get get hit. The higher PRR and HP boost will matter

do this for the HC server - see how many players it has taken from the regular servers

I believe hand wraps are considered an exotic weapon
They could have just not put in the old guild amenity vendor on the HC server
