Just found Epic Holistic Stave on patriarchs crypt setting the wards did not see it on the wiki list. If there is a thread to post images or info for the wiki I did not find it feel free to direct me to it for the future.

There were some issues with that instance's chest not spawn initially after they changed the catacombs loot from being an end chain rewards list to end chests. A patch or two back they fixed it so the chest would spawn, but it seems like it's still slowly trickling onto the wiki as people find the loot in there and make edits (or post about what they find).
