What method are you using to target it?
IIRC the Tab key cycles between monster targets, and the Q key by default cycles through interactables targets. Are you using Tab?
"Auto Targeting" in this game means that it will select as your target anything *that you are pointing your cursor straight at*. It doesn't "hop" the target automatically to whatever happens to be nearest, you still have to actually AIM at it. If you turn auto-targeting off, it won't select the thing as your current target. This can be helpful if you're trying to maintain targeting only on a specific selected target, such as when you're healing.
Use the keyboard shortcuts (Tab, Q) to cycle through POSSIBLE targets without aiming at them. Then you can use E to automatically interact with it (if you're close enough).

You may have pressed to key combo that disables this. It happened to me a few days ago. I don't remember the default, but go into Key binding and look for toggle auto-target. If key is assigned just press it to get it back. Otherwise bind a key to it then press to toggle back on.
