Just ran this quest on Lamannia, to see if it was fixed, and sadly it is not.
In between mobs, Amalgam attacks Heyton, as if he was an enemy. Without healing Heyton, he will die no matter what, from these attacks.

Not sure. I ran it with my PM, at level and it happened. One of the waves comes almost right after the last enemy of the previous wave dies, and Amalgam still hit Heyton, right as the new wave started appearing. So I'm not really convinced it's an issue of killing the waves too fast. Although, I suppose it's possible, but it certainly took me longer to kill them, than it would if I had been using a THF Barb (at least for me).
But there are a few other threads about this issue, I just decided to check with the new update on Lamannia, to see if it was fixed or not. Here's some links to a couple other threads about it.

When I've seen this behavior it has not been "keeps attacking Heyton till he dies". It's just been that the two of them, after a round of combat with sahuagin, keep swinging a time or two after the last Sahuagin is dead, and one of them seems to connect with the other. (That is, I can hear the noises associated with swinging and hitting.)
While I personally have never had Heyton die as a result, it makes sense to me that it could happen if he was very low on hit points.
Also, I think I might have seen similar behavior in another quest with NPC combat going on. Can't remember for sure, much less which other quest it might have been. That means it might not be a bug with this quest, but rather with the NPC combat routines in general.
i have seen this happen a few times and bug reported it (nearly a year now?), haven't seen it on the reported list.

What seems to be happening is that Heyton gets damaged by amalgam's great axe though not by the full damage but by a percentage (maybe the glancing blows damage?)
Didn't this happen years ago to players?
Wasn't it fixed back then?
Module 1 Patch 3: The Harbor
Players will no longer accidentally kill each other with two weapon fighting and glancing blows.

New to DDO and this happened to me. I went in with a Rogue. Seemed to have beat back the hordes when one of the good guys started attacking. Failed the quest and not sure I want to try it again.
Bug report it, hopefully it will hit the list.... one day...
For now, bring a healing hire. buff him with aid (found on bracers here)
Yeah I think I found the problem and fixed it, but we held off on putting it in U20 to do more QA testing : )
I got a bug specifically about amalgam hurting lars, but it appears that all player-allied defenders actually hurt each-other with AOE attacks (most most wide swings)
I should have had this added to the known issues.
If the fix is successful without any side effects, you should see it in the next patch!

At least, I'm hoping they're related since both issues showed up at the same time, and sound similar in nature.
Ah yes, this reminds me of a time when I used a Summon Monster V scroll that I baught from the guild vendor while in The Bloody Crypt to summon a bearded devil. Like you said, whenever he uses a AoE attack, like his spinning cleave, he actually damages friendly NPC's, such as hirelings. I thought it was funny to see friendly fire like that in the game.
