Finally, make sure you completely clear your old quest before exiting the lfm window, and always completely clear old quest lfms before starting a new one.

Also, you can avoid this bug simply by exiting mouse look before upgrading your lfm.

This is not a bug. To correct this, use your keyboard 'I'. It will force you out of mouse look. Then, you can use your mouse to close this window.
