To note they added monitoring. Monitoring means they are "watching" for the next occurrence so they can "learn" what is happening.
This usually means they could not re-create the situation in a vacuum. Either the vacuum is not calibrated correctly or they are missing details to calibrate it.
I am saddened to see loses like this.
While there is no definitive proof only antidotal that logging out of the client before doing a Reincarnation, I would point out that if you know of something that might give you a better chance of not taking a loss (such as locking your front door when you leave home) it does make sense to consider those options.
Now they didn't go into details about monitoring. And I'm actually sure they are not sure what they are looking for as they seem to still have been dependent on users notifying them when it occurs.
As a troubleshooter type, my guess is that there is an issue in the TR CACHE Loading. As the OP mentions in one of the posts they still had all their backpack slots and these were "overfull" and what they had in their bank did go to TR CACHE.
So the question is, of the failures that occurred after the monitoring was put in place, did DDO development gain any insight on what occurred? To me that silence is the scary part.
The one time I had a reincarnation issue (I lost tomes on an ER, fixed by a relog while still in the bridge between worlds) it definitely had to do with loading/lag. I could see that all the gear I had on was still in equipped slots redded out, you can observe this anytime you do it, it will be equipped then processed that you can't use it anymore. Since then I do not even try to move until I'm sure it's done loading and have always relaunched before any reincarnation.
I would first like to say how sorry I am for all the time and effort lost with your gear. Thank you for sharing it with the community, as I'm fairly certain some of us were lulled into complacency with the introduction of this tracking system.
I have every belief that SSG's tracking system is real but doubt highly that it was introduced to serve players by providing a fail-safe on TR gear losses. It was introduced at the height of the 'issue' they were having with the Otto's Box Stones. I suspect it is tracking TRs, just not gear. I would love to be proved wrong and that you will get all your gear back once SSG is back on Monday, but I'm not gonna hold my breath.
Thank you for identifying yourself in your post. You are on my server. Despite being totally burnt on Slave Lords after grinding out 10 pieces, I'd be happy to help you farm a new set.

The fact that two people in my guild have had the same issues with TR'ing shows it is a common problem. Our guild is an end game raiding guild, many of our toons are completionists and we are tr'ing now for racial lives. I know the gear I have on my main I have accumulated over almost 10yrs. How can you replace that? The time and effort that we put into our characters is tremendous. Then to have SSG basically say sorry here are a few TP and send you on your way is an insult. Fix your shi.. stuff SSG.
TR tracking and item tracking is in place, it's just not all items for each character so it seems.
I think I'm going to build a list of items I have on my main... with screenshots.
I to have had some trouble once with tomes and such lost on a tr, luckily for me that was sorted out by a GM rather quickly.
(So I learned the hardway to follow those TR-steps SSG gives. :( )
I don't know why they can't do a restore. If you delete your character, you can pay to restore it. You might lose some recent progress but at least you have your items. We don't know how far back saved copies are kept but I would be asking for a free restore.
ALL of the hearts of wood I had on one character after TR are gone. Since 06/2017
support ticket and bug report submitted, havent heard anything yet.
Honestly at this point I'd like them to do a deepdive and figure out the tr cache. Either fix it or replace it with something new and grant players increased bank or inventory space to compensate.
This should be a #1 priority at the moment even moreso than ravenloft. The tr system like it or not is so much apart of this game for better or worse that having it not functioning properly in such a way as to cost players real money and hundreds (in some cases thousands) of hours of ingame time is one of the worst affronts a gaming company can do.
Yeah, but that's like a 3th line solution, you know... the one that actually involves doing work.
And for the Guide, my search foo is weak today and I don't know if it been mentioned in one of video casts or as a post somewhere on this forum, but yeah there is a step by step instruction written somewhere for tr-ing.
It's not that difficult, you just have to know when to shut down DDO, start it again and explicitly not what to do in between.
Yeah, as OP mentioned (and I said in another thread, when it happened to me), logging out every step doesn't even guarantee it. Maybe it helps (we'll never know), but it's not 100% for sure.
That hasn't mattered in the past for GMs restoring gear. I suppose it'll help you know what you lost, but they don't count it as proof of anything - you'll get the same response as if you didn't have any.
In any event I don't think the player can be blamed for something that should be working. However, I used to have issues with destinies and tomes during the TR process that were always resolved by support. Since I started waiting for some time after getting the messages I was ready to TR and then doing a full reboot - I've now gone hundreds of trs without incident. Of course it's not enough to indicate a cause/effect I think SSG should look at whether fast shutdowns can contribute to the problem. I am believer that taking the TR process a little slow is a good idea, but that doesn't rule out completely independent issues impacting lost items - I never had that issue.
