KotC Cleaves not working correctly

I can't use Avenging/Exalted Cleave in a dungeon, but can use them in a city instance just fine. This started happening yesterday.

Which dungeon(s) did you run into this issue with? What weapons are you using? And are you sure your hotbar where these attacks are on is the "active" one?

All of them, long swords, and yes I even respec’d and reset bars, same behavior

I was able to use them on my pally in an epic Eveningstar quest an hour ago. Using greatsword.
