What I am using and what I plan to upgrade:
Current Keyboard: Tesoro Tizona
Next Keyboard: MAX KEYBOARD BLACKBIRD TENKEYLESS
Current Mouse: Tesoro Thyrsus
Next Mouse: Roccat Tyon
Current Footpedals: Generic 3 pedal USB footpedals (Like a cheap $30 model)
Next Footpedals: Custom Arduino 3 two way pedal setup (Total of 6 programmable buttons)
Headset: Logitech G533 (Perfect for my needs)
Gamepad: Azeron Compact with glow in the dark keys (Perfect for my needs and highly recommended)
So, I am curious what all you guys are using and would be especially interested in hearing what the most elite players are using :)

I was going to joke about using a footpedal, but I see you already got that covered. :D
Maybe a steering wheel for those full-sprint quest completions?

Basic Logitech mouse... but it's red.
And a POS Logitech set of headphones/mic.

Headset - Steelseries Siberia V2
Keyboard - basic logitech wireless
Mouse - logitech mini travel mouse, I prefer a smaller lighter mouse
So nothing special other than my headset which is decent
Foot pedals I don't have, though it'd be helpful on my cleric atleast. Just haven't wanted to have the clutter under my desk

For me:
- Standard Laptop keyboard; I have no reason to upgrade this.
- Skullcandy Crusher headphones, though I only use them for certain quests that I really want to enjoy the audio of
- Elecom HUGE Trackball mouse; a trackball really gives me the maneuverability I need, for DDO, shooters, etc, and this one has tons of extra buttons.
Question: What do you generally use the foot-pedal for?

I've found it to be phenomenal, even better than the Logitech trackball it replaced. This guy does a great review of it if you're curious.

Keyboard: Microsoft Sidewinder 6 (entire numberpad is detachable and can be moved to left side, all numberpad keys are programmable). Has a bank of 6 programmable keys (in addtion to the numberpad), which gives me like 24 programmable keys). It's been discontinued, however, and I have NO IDEA how I can replace it since I can't find a keyboard with 18 or so programmable keys (had an OG Logitech G15 before with 18 G keys, but that version is also discontinued and the new ones have 6 keys... 6?!?!).

Ah, and here's the caveat for you: the one I linked does not have a 10key keypad -- not even a "virtual one" where you reuse other keys as a 10key. This is bad if you use alt-codes under Windows; it needs Alt+Numpad to access letters used for languages other than English. I only do that a couple times a year, but even then it's irksome to cut'n'paste from web sources instead of alt-coding it.
It also claims durability, but I typically replace about every 1-2years. Why keep buying? It's 100% purely for the shape, particularly for the built-in palm-rest. I kick back and type with the KB in my lap. BTW, breakage may be my fault; decades later, I'm still a loud and forceful typer from learning on a model 33 (https://en.wikipedia.org/wiki/Teletype_Model_33). You could actually put your hand on the keys and lean your weight on them and they wouldn't move.
On the bright side, Corsair's are the only wireless I've ever used that don't loose keys due to typing rate. I can't tell the difference in responsiveness between wireless and wired. Swapped to wireless last buy and it's nice.

Default keyboard (Mac Powerbook, base 2015 model)
Basic Logitech USB mouse (blue; the color is SUPER important) with one corner's pad rubbed off so it inadvertently makes a skritching sound on my wooden dining room table. Google helpfully tells me it's $13 at Staples!
Plug-in earbuds that I found lying around and, upon further Googling, are apparently about the same price as my mouse
Bog-standard USB charger so I can pull up DDOWiki on my phone for hard-to-navigate quests, since this potato of a computer doesn't like screen-swapping much
Ph33r my l33t setup! But in all seriousness, one of the things I like most about this game is that you don't have to have all the bells and whistles to enjoy it.

Steering wheels are a waste of space, I use footpedals for emergency effects such as teleport and dimension door as well as raid essentials like removing grip effects, curse effects or in the lord of blades raids mass unbreakable forcefield for when the daggers fall.

If you're wanting serious.
Mouse: Razer: Naga Left-Handed (https://www.razer.com/gaming-mice/razer-naga-left-handed-edition/RZ01-03410100-R3M1) - nothing else performs close, especially if one is left-handed.
Keyboard: PI Engineering: X-keys XK-60 USB Keyboard (https://xkeys.com/xk60.html) - Mechanical, fully hardware (which is what you want) & software programmable keyboard. The primary advantage of these keyboards is they are built for workflow, not typing. So one defines all the key strikes according to one's gaming workflow, which overrides even games which don't allow key rebinds because every key is written.
It doesn't matter what keys the game itself binds to because you can create a hardware (or software) profile for the game and rewrite the keys to the keyboard before playing if you want to swtich. I got years out of mine before I wore out my first (one key really) and needed to repurchase another.

I can't find a keyboard with 18 or so programmable keys
Look into engineering companies that build programmable keyboards for industrial applications. That's where I find industrial mechanical keyboards going up to 128 keys, fully hardware programmable (aka no software involved).
The programmable features on the engineering stuff are unmatched. Unlike the way most consumer-facing keyboards allow reprogramming, they enforce separation of all components of the key strike (down & up).

Keyboard - Razer Blackwidow Ultimate 2016 Edition, works fine going to keep
Mouse - Logitech G Pro Gaming Mouse, took a bit of fine tuning to get the mouse and keyboard the same color, I am going to stick with the mouse
Footpedals - I never saw a use for them in DDO
Headset - Sades A6, old and have not used them since my fairly static group all stopped playing years ago
Gamepad - That thing you use also looks like it is part of a bondage set

Mouse: Logitech G600. I've traditionally used the Razer Naga, but after 3 Nagas I tried out the Logitech version. The Logitech is half the price and feels looser/cheaper, but in use is functionally the same and has shown to have the same or perhaps better overall durability. I just bought a second d G600 as the first one is just starting to fail after about 2 years.
Headset: Logitech G533. Complete coincidence this is Logitech. I needed a new headset and this was on sale. It has performed above expectations so far.
Keyboard: Logitech G510. More coincidence that I'm running triple Logitech, but this thing is old, I mean old, and keeps on ticking. The extra LCD display is mostly pointless now, but the keyboard works. My only gripe is size, the only reason I haven't replaced it with something smaller yet is because I haven't really seen anything that quite fits what I want perfectly. I might just build a basic ten keyless mechanical to replace it at some point, but it needs to be on the quiter side of mechanical.
On the plus side running triple Logitech means one software suite covers everything.

The one thing I'd really recommend people spend some spare money on if they have it is a good headset. The difference between a cheap $15 headset and even just a mid range $75 headset is night and day in terms of both quality of sound you hear and the quality of your voice going through your mic that others hear. Easily worth spending a bit more money on for anyone that uses a headset for any significant amount of time.

i run audio technica air ATH-AD700x headphones and a blue snowball mic. they are open backed headphones so i can hear whats going on around me, if someone needs help or whatever i can hear it even if my sound is turned up, this is a plus for me, but might not be a plus for you.
Steelseries Rival 500 mouse. i think this one is made for moba games, but i like it for ddo, not so many buttons as one of those razor mmo mice, but each button feels different so i dont miss buttons ever, and the optical laser tracks wonderfully for FPS games.
logitech g710+ (orange version) with aftermarket high quality keycaps. this is the quiet version of a mechanical keyboard, its still louder than normal keys, but im not keeping up the whole neighborhood if i play at night.
for screens, i have an asus 27" oldschool screen (1080p, 60hz) on the left for youtube/browser stuff, in the middle is a 3440x1440 ultrawide at 100 HZ with freesync for games/whatever and on the right is a portrait mode benq 27" screen that is only for discord (discord in portrait mode is amazing)
my PC is a homemade about 2 years ago system with an i7 8700k, 16 gig of ram, a GTX 1080ti and a few SSD's and a few HDD's. the main drive is an m.2 1 TB samsung SSD that goes at like 1200 MB/s. with current PC part pricing, im priced out of any video card upgrades for a while it seems.
