Falling into lava point of no return in the madstone, but being resistant to fire dmg>50 ad having a passive regeneration. While lava does not harm you, you have to leave the game to allow the server arbitrarily decide you are killed by the lava, which at least lets you to leave the quest zone. Yet again, it is HC so you are dead, DEAD.
