Been playing for a while (24th on 1st character, 17th on 2nd character) but have no idea how to get a +20 Lesser Heart of Wood. Went through the forums (search function) but couldn't quite figure it out. Can someone please list all the ways to get / buy one? (It's not currently in the DDO store...is it ever on sale or does is have to be a random loot drop or daily dice result?)

+20 Lesser Hearts are a DDO Store item that gets put up on a very occasional basis, but are not normally available for sale. Under normal circumstances you need to buy and use multiple lower plus hearts to achieve the same effect (and you can only LR every 3 days).

In nearly all instances, it is better to TR than do a +20 heart.

So, IF I wanted to replicate a +20 Heart of Wood I could do it with four (4) +5 Hearts of Wood? But, would have to wait MULTIPLE days to pull it off?

Yes, seems like it would be best to just get to level 20 and then TR...correct?

Correct: there's a 3-day cooldown between LRs so it would take you nine days to finish. [LR #1 -> wait 3 days -> LR #2 -> wait 3 days -> LR #3 -> wait 3 days -> LR #4]

Unless you're a super-slow leveler - and maybe you are! I'm not one to judge - yes, that would be the simpler (not to mention much less expensive) option.

An admin for DDO must be listening because the store just put on sale +20 Lesser Hearts of Wood. YEAH!!!
