I was playing DDO normally two days ago, then logged in and my controls were all messed up.

In a couple of seconds I noticed:

left mouse button was no longer Interact (now it's RMB)
move forward/backward was inverted on my gamepad
move mouse cursor up/down was inverted on my gamepad
somehow Esc got mapped to a button on the gamepad
all other button assignments seem to have been removed

Anyone know of a quick way that I can restore my keybindings, or do I need to do it all manually?

Sadly, no. In the future you can make a copy of the userprefences file. Its located NOT in the game folder, but on windows, in Documents, Dungeons and Dragons, etc.

Have you tried clicking on one of the style buttons, such as FPS? ESC, Options, Keymapping, and there are 3 choices just under the Keymapping tab that quick set, default, fps, classic.

Sorry not more help

Try going into the options keyboard section and changing to classic or role playing mode. There are tiny selection buttons under the main selection. I have had occasions where it suddenly required a double click and I returned it to normal by first going to classic mode and then resetting to how I use it.

I feel your pain. Happened to the wife and I a few weeks ago. Clicked the default. And it straightened everything out. I do not know why this happen. I chalk it up to a game engine that is over ten years old with little if any real update.

Thanks for the replies.

I'll be sure to save my preferences file separately for easy restoration.

Unfortunately, I can't just press a button and get back all my keybindings, because I use a gamepad and have to customize every key assignment--DDO doesn't really have much direct support for a controller.

In random intervalls my client resets itself.
- it switches back to direct x9
- I lose all my keymaps and preferences
- and it starts downloading the german client.

Luckily I saved my settings but this gets annyoing. It happens every other week but at least 2 times in a month.

Some suggestions?
Thank you.

right click run as admin works for me to fix it

Thank you.I've seen this, generally when trying to reload the client before it's finished unloading with a lot of built up memory leak. Best bet is to open task manager (if using windows) to ensure the client is done unloading if this is also the case for you.
