Any dungeon where the map is pretty much useless - the Road to Tarath Marad (Servants of the Overlord) comes to mind.

Anything that has to be done without killing things while I'm playing my Fire savant, the passive fire shield effect makes it tough/impossible to do them because I just passively kill anything that attacks me.

Any dungeon where the map is pretty much useless - the Road to Tarath Marad (Servants of the Overlord) comes to mind.
My fix for not getting lost in this one (assuming I am thinking of the correct quest) is when you get to the point where the crests have to be placed, step through that first door and go right. Always, always right. You will eventually get the crests and the optional and end up back where you have to put in the crest. After that it is pretty much a straight line.
I always get lost in Coal Chamber. I despise that quest - there is a reason we call it "The C word."

the quest I get lost in every time is Reclamation. In addition I almost always fall to the bottom on first entering.

There plenty places I get lost in but would have to say the place I find myself having to check map and figure out which way to head the most often would be in the marketplace. Only because I am in the market place a lot more often then I am anywhere else other then maybe the guild ship

everyone who knows me laughs at me for this, but... Fleshmakers. the only reason I am getting lost more often in fleshmakers than in Inferno is that occasionally i might run Fleshmakers twice per life, whereas Inferno I often skip completely... yeh... I am really no good at necro 4.

What dungeon do you tend to most often get lost in?
Disclaimer = I'm a relatively new player and I'm fickle. i.e. roll a character, messing around to level 6 or so. Get bored. Delete. Reroll.
That being said....
Answer = Misery's Peak... in specific... those spider platforms. I don't think in 3D so, I go up and down and then right... and now I'm lost. And since there's respawns... I can't tell if I've been here before or not. And that's after doing it over 30 times (!).

I used to have problems with Trial by Fire, but I learned it. Same goes for the Pit. Also Trial by Fury was first confusing, but not last time, but I keep missing some optional areas.
I think the worst now for me is Caught in the Web, as I can't solo it to learn the map.

I get turned around in two quests that look a little alike: Bastion of Power and The Devil's Details. Neither of which I run very often, so I tend to lose whatever I re-remembered from the last time I ran them.

None. I intentionally explore the same area multiple times to ensure that I did not miss a collectible, breakable, or mob during my first 5 passes! :D

I most often get lost in The Pit, but I found I can go in and before starting any of the rooms I can walk up and down both paths unmolested and chart the relative position of the rooms to the paths and then run it with ease. There are only 2 paths, all rooms are along these 2 paths as the paths curl around 2 central columns, and the 2 paths intersect once

Well, I tend to get lost in 95% of the quests and most wilderness and public areas. However, because I am in there the most, I would say I get lost most in the Cannith Crafting Hall quest.

I don't tend to get lost in quests.
If I take "lost" to mean ones where I have the most difficulty finding an objective, probably The Enemy Within.
