I'm getting real tired of beating down creatures that either burrow (scorpions) or disappear otherwise (wraiths) and then not only heal up to full, but reappear as a champion. It is making me want to stop playing DDO.
Thank you, that is all.

NOPE! At least not everywhere.
Try soloing The Claw of Vulkoor at level on reaper, hell I even tried it on elite and you will find that out fast enough.
I made it to the last barrier before I just gave up. My little Rogue would die, he'd run his tookis off and barely make the shrine, he'd heal up and head back to the fight only to find all the scorps healed, not a surprise I got near instant red alert (mobs aggroed on the other side of barriers) and there were a ton of healing scorps in there. What was a surprise was to find that the 2 champs were now 5 and the orange named scorp was now a champion. I died a couple of times but when I finally gave up I still had about 10 scorpions to deal with (2 orange named) and all but 1 were champs. When I started that fight I had 1 healing champ and 1 roaming. both orange names were not champions when I started.
At least they aren't ending up with ALL the champion buffs though. I was a little too busy to tell if the buffs they did have were changing when they resurfaced though.
Basically I just gave up running the fens entirely (usually run the chain as I do enjoy it) as I couldn't find anyone interested in the LFM and though I could solo everything else the claw was just plain not going to happen. The "stealth" run was impossible even though I have that maxed out something is borked there so Red alert and wipe. I then tried a nibble at them and kill your way through it type of run but aggroed a ton of stuff on the other side of a barrier and got zerged. I can deal with champs for sure, even 2 or 3 is something I can solo manage on an Arty Rogue great crossbow guy but half a dozen or more, I just can't do enough AoE dps to manage that. Kinda frustrating to be unable to complete a quest even with a hire and pet that I have solo'd many MANY times on many builds if I am being honest but it just means it falls out of my completions unless I can find a group moving forward.
Sure would be nice if they did fix it one day though.

Ive been doing some Dreaming Dark farming and the Dream Stealers keep doing that. Get them down to 10%, they fade out and reappear healed.

If you want them to fix a bug, you have to tell them why it's a bug that benefits the players. If it doesn't benefit us, it'll get put on the back burner, likely to be left to burn to a crisp before being taken care of.
With that having been said... *clears throat*... Devs! Please don't fix this bug! More enemies coming back as champions means more remnants for us! THIS IS THE MOST AWESOME BUG EVER! I'M LITERALLY SWIMMING IN REMNANTS THANKS TO IT! DON'T FIX! DON'T FIX! EVERRRRRRRR!
There, that should get the job done.

Ogre Magi are doing it all over to. Just ran a slavers of the Shrieking mines and had 2 ogre magi disappear (not champs) and reappear as champs.

Yes. Sneaking toons is 100% ruined in several of the dungeons, now, due to the forking "aggro" engine.
I just did, at level, Sharn Syndicate, where you supposedly can sneak into the main bank past all the guards, and flip the switches.
I have a max-sneak toon too. (but no past life uber-gear)
It ought to have been dirt simple to sneak past the guards and flip those switched, turning off the alarm.
Nope. Instant Aggro: the entire room instantly spots my max-sneak thief and begins pounding him to chum.
Why even have the option to NOT kill the guards, if they *all** can literally seen invisible?
Stupid-stupid-stupid. Listening to the exploit "players" and ruin the game for everyone...

LOL. With consistent examples like the ones listed below, you take them at their word that they fixed it?
https://www.ddo.com/en/update-33-release-notes
The entrance to the Gianthold wilderness area no longer sometimes takes you back to the Gianthold public area. - Nope. There are still two zone entrances near the Ruins of Gianthold. One takes you to back to Gianthold, one takes you to The Gianthold Ruins.
https://www.ddo.com/en/update-33-patch-1-release-notes
Red and Purple named enemies can no longer erroneously spawn as Champions. - Nope. Red mobs are still appearing as champions.
https://www.ddo.com/en/update-33-patch-2-release-notes
Monsters should no longer get aggro through secret doors. - Nope.
https://www.ddo.com/en/update-34-release-notes
Red-named bosses can no longer be Champions. - Nope. Third try was not the charm...
https://www.ddo.com/en/update-34-patch-2-release-notes
Warpriest's Magic Backlash Tier 1 description now correctly states in its tooltip that it does not trigger off of Positive spells. The remaining tiers continue to list an inaccurate description. - Really? Way to give up 5ft from the finish
https://www.ddo.com/en/update-34-patch-2-release-notes
Chain Missile's long description now properly state how the spell actually works. It now states: "You gain an additional missile at levels 4, 8, and 12 for a maximum of four missiles." Note: The short description continues to erroneously state that you get an extra missile for every four levels. - Another quit right before crossing the finish line...
