I recently looted a level 16 item with +spell saves.

Does anyone know if this stacks with +resistance bonus to saving throws ?

What is interesting is that I can confirm it does not alter spell resistance, or alter saving throw scores.

So either its bugged and does affect one of those scores, or it stacks with something but i am unsure what...

Anyone know ?

It is in addition to your basic saves (ref/fort/will), but only versus spells that require a save, not effects. As it does not affect your saves in general it is not a listed bonus in the char sheet.

RD2play's answer is a tiny bit ambiguous, so I'll fill in with a more exacting description.

RD2play is correct in that the effect from Realistic (+X Resistance vs Spells) apply only when a spell effect is triggered on you. The bonus it provides will not be displayed on your character sheet as the bonus only triggers under certain conditions (being hit by a spell that has a Save roll to it). Which is why you will not notice the bonus it provides unless you test it out thoroughly.

Description wise, yes, its description says "Reistance" bonus. Which in theory shouldn't stack with the "Resistance" bonus from the other items such as Staunch, or Resistance +X items, etc. However, I can confirm that despite this, current gameplay tests show that this Resistance vs Spells bonus IS being applied (stacked) on top of your saves if you are hit with a spell that has a save component to it, even if you already have an item or effect that is giving you a "Resistance" bonus to saves already.

As to whether this is working as intended, or whether it's a bug and will be fixed later, only Turbine knows.

I can second this, even if only because certain items (see Slave Lord set bonuses) contain Resistance AND spell saves independently. Seems it's quite deliberate that they stack.
