You can’t enter The Tide Turns quest while in a party. Need to relog.
When you enter the ship in the same time as an another guild member, you will end up on an empty ship.
The AI seems to be borked. The monsters often fall off the cliff when aggroed causing many optionals and quests to be uncompletable.
When you open the EDs’ sheet while below level 20 the message You cannot change your Twists of Fate while under level 20. x4 will pop up on your screen and in the chat. It will repop everytime you change the instance until you relog.
The party chat often splits into the multiple instances causing the players not seeing each other talking. After someone disconnects or after someone joins the party that has been formed for a while.

CiTW: After destroying the 2nd optional artifact, it is common for the portal (way out) not to spawn.
Bladeforged, Sun Elf, PDK and Shadar-Kai don't have their own icons in the Social panel.
Ghost LFMs.
The LFMs won't show the party leaders and members of the parties unless you open the Who list and let it load.
Cry for Help: If you pull the two levers lowering the bridges in the same time, it will result in the bridges not lowering and the levers not targetable. Need to restart the quest.
TWF offhand attacks and shield bashes do not graze, but rather miss outright, even when using a weapon you're proficient in.
In order to get the 3% spell power bonus per past life from Arcane Past Life Stance you need to turn it off and then on at each login.
The Haunted Halls: The Doppelganger fails to spawn when the crest lies on a pedestal without the statue. Need to restart the quest.
When going to the 3BC from the ship, there is the Restless Isles loading screen instead of 3BC one.
When finishing out of the Don't Drink the Water quest, there is the Sschindylryn loading screen instead of Eveningstar one.
Force and Critical Force Rituals do not work on throwing weapons.
Ioun Stones, Dusk Heart, Dumathoin's Bracers, Shadowscale Outfits, Dynamistic Quiver, Quiver of Alaricity, Cloak of Invisibility, Xachosian Eardweller don't have the named item borders on their icons.
When you acquire the Vault of the Night pre-raid from the NPC, it plays 5 quest completions in the same time making the sound very loud.
When reincarnating, all the channels and all the comments in the friend list will be lost.
Trial by Fury: Grulemith's jump attack in the end fight causes the lag spike.
The Legendary Shroud: The monsters in the end fight have the champion crowns even though they are not champions and only have a special ranged/melee power boost ability.
The Adventure Compendium loses the order (by name, by level, etc) given by the player at each closure.
Completing the War of the Archons quest arc on epic gives the heroic reward.
The Elemental Arrows (Corrosive, Flaming, Frost, Shock) from the Arcane Archer enhancement tree get turned off at each log.
The character sheet list doesn't show properly the stats over 100.
There are no more Turbine points rewards after the player reaches 5000 favor.
Many favor rewards are missing/uncomplete (LIST: TBD).
Completing The Chronoscope on epic gives heroic low level randomly generated items.
The monsters stay aggroed on the player who died.
The Prisoner: The Quori Stalkers in heroic version hit the players with unreasonable charisma damage (probably swapped with epic).

Ah, true, forgot about shield bashes.

It might be WAI, who knows what the reasning is. I must say, though, that in that case, I seriously don't understand the idea of the combat system. Well, even less than I currently think I do :P

This is not a bug but done on purpose. This used to work correctly, but was one cause of the inability of people to get in the game the first time. You also don't have to let the list fully load, just clicking the who tab, wait for it to start loading, and then you can switch to the LFM panel.

If you want to change it to something about retrieving the who list in the background to stop issue related to it, I am fine with that, but what you have is WAI.

- Insightful Assassinate on gear can still produce +0 effect.

Not sure what else out there is still in the Zero crowd among new random loots.

Hm, when did the LFM panel break that they had to auto-load the who list? Before 2012? (my join date)

Because I remember the story differently:
In Update 23, they decided to "fix" the double login issue as well as the no login issue (where you would disconnect every time)

The measures they took were:


The DDO game launcher now has the option to "preload" the client_gamelogic.dat files, which can speed up initial game load and resolve an issue that could cause players to face an initial dis-connection to the game world upon first log in. Players currently utilizing third-party tools should no longer need to use them. This functionality is enabled by default if you have more than 2 Gigabytes of memory on your computer, and can be disabled in the game launcher options (under the heading Pre-cache gamelogic.)
The Who list will now load the first time you select the tab, rather than during character log in.


So that's when the LFM panel broke. You needed to switch to the Who panel to load the players or you would be seeing ghost LFMs.
Next update (Update 24) included 2 "fixes" for that:

The Social panel now defaults to the Guild tab after initial login.
The Looking For Group panel now loads groups upon initial open.
The panel defaulted to guild tab because if it remained at group tab and the who list was loaded upon initial open, then you would load the Who list in a possibly unwanted time (perhaps when you tried to see something in your guild tab).
However, the 2nd fix was not the fix that needed to be applied. The groups did not have a problem, the players in the groups did. It wasn't the group list that needed loading, but rather the player list.

So, till today this remains unfixed (and not WAI).

Yes, I think you're right on that.
I don't remember what update exactly the LFM broke, but around U24 sounds right, related to the attempted fix for the login issues.

I'm pretty sure though on the part that they implemented the automated loading of the who list as a workaround, to fix the broken LFM panel, which caused again trouble on it's own and was therefore later scraped again.
