You killed or stoned the runts before they rang the warning bells. It's possible you didn't get close enough to trigger some of them. The runts are the only ones (other than players) who ring the bells.

you probably killed the scouts quickly. They raise the alarm. Either sneaking or killing them all first & fast will get you the no-alarm optional.
I do not think they raise it if you invisi run by them but not sure.
