I have rarely experienced lag in Tempest's Spine on any difficulty. Perhaps this is tied to a combination of the population of the server you play on and the other characters in the party? Try to take some notes on which characters are in your party, what their race/classes/levels/active EDs are. Those details can help "figure it out" some times.

There is one spot that lags really bad at the Red named Drow, if someone rushs in and pulls all mobs. Lag sometimes at main boss. But that's about it.

Tempest Spine has no coded lag, so it's likely on your end. When you say lag, do you mean actual lag where you get glued to the floor, or do you mean graphical stuttering where the game becomes a slideshow?
