How about creating environments of quests affected by outside occurrences in the game world like when it is raining a certain monster appears, if the moon is full an item has a chance to appear etc., and carrying over the time of day and conditions of the real world into quests?

Considering it ahs a global base, carrying over the actual time of day and conditions of the real world is basically impossible. Only the people living in Boston would have any correlation.

I meant the real world in game, not as is in quest.

really cool idea! That would accord well with Druid season herald enhancements.

Considering that there are something like 3-4 game days per real day.... (seriously)

and wasn't there a quest in the past where you had to talk to House Orien guy in harbor at midnight?

This is already partially implemented in some areas. The first ones that pop into mind are some of the rares in both Kings Forest and the High Road.

Kings Forest: rare/random encounter: The Hunters Feast: can only be completed at night (otherwise the hunters just sit there and nothing happens... note: must be actual night time in game. The Darkening drow magical effect does not count)

The High Road: Frozen By Night: can only be completed at night (when the alpha werewolf finally shows up)

Though I do agree, having more of these, and some based on weather (like rain) as well would be nice.

Great points. Consider also how it might impact facing undead. Running around in Orchard during the day could mean no vamp spawns. I think the more that this idea is considered, the more we would experience a sense of immersion in the gameworld.
