OK, I have not had any solution and its driving me nuts. I tried to submit a help ticket in game and I just get 404 errors
If you can help me fix the fact that my keymapping keeps resetting every time I close the game I will donate £20 to a charity of your choice (and you also get to help out the other people with this problem)
Im on a brand new Win 7 and DDO install

Have you tried running the game with admin rights, change the mappings, then log out? use the same account you use now, but give it admin rights. If it already has admin rights, try right-clicking the launcher and selecting "Run as admin".
I have no experience with your problem, but this is the first "Windows" solution that popped into my head.

Make sure that you have Write Access to both the directory where the keymap file is stored and the actual keymap file that is there already. I think it is in Application Data so you you should have access, but this sounds like it can't persist the altered keymap file which would usually indicate a file access related problem.

An issue I noticed was I had to remap the defaulted key to something else then map the key I wanted to the effect.
Otherwise when I relogged the original defaulted keymap would reassert itself.

I tried setting file permission yesterday and I gave all right holder on the pc full access to write the keymapping and the other file (preferences?) and it did change anything
Think fedora is close! When I run the game as Admin, it remembers my keysetting. When I run the game normally, it uses the blank keysetting

Did you try editing the file with notepad and saving it, just to make sure everything is lined up? I think its a text file, so you should be able open it and save it with notepad, insuring that its saved by your current user id/process. Your symptoms still sound like a permission issue.

You need to go to control panel, users/accounts, and give your logged in account admin privileges.

I have checked that and I am an admin. I given my self admin right including using the hidden win 7 cmd line super admin rights

If that is the case, nothing should be blocked. How low is your UAC levels? Maybe lowering those may help (just to test). And you may try opening an admin command prompt and trying attrib -r on ddo's folder and keymap file. Or delete them/move them to the desktop and relaunch the game. If any file is damaged or corrupt, doing that should rebuild them.
