if I slot 2 of those into a weapon that has 2 slots, would the effect occur once or twice?

Nobody knows since it's a new item and Mabar hasn't opened yet. (there might be on or two people who picked one up during the preview)

Although I could speculate and say no, it shouldn't.

If I was going to speculate I would actually say it would since two of the same type of damage augment stack.

Well... Ruby of the Endless Night procs on vorpal hits. The only vorpal effect I had tested for "double proc" was Smiting bow with Smiting AA arrows -- and it does in fact inflict extra 200 damage on 20's. So my guess is yes.
