Underdark Dragon?

Does he exist?

I've visited his lair more than ten times, and he's never there.

Is the chance for him to appear low and I've had bad luck?
Do you need to do something specific to get him to appear?

I've seen him 3 times and don't think I did anything special. I've been to the lair many more times though.
