Joining an EE run with my barbarian without putting my heavy fortification gear for 15-20 minutes without noticing. That was a meat sack that got alot of insults.

~Equipping TOD boots as Horoth drops, get banished... as you are complaining about having your boots on...a moment of realization hits as you are featherfalling...
.. Doh... Cannith Boots of Propulsion.
~Repeatedly hasting myself after a TOD run and then realizing haste isn't broken...the boots are still on....Doh...
~Con belt is not a good swap location for heal scroll UMD users and Planar Gird GH...Ding...
~EDQ endfight and Autorun = Bad... Forever thereafter Disabled Autorun.
~Watching Lando demonstrate strafing to newbies at the wait point in Abbott and starting the fight with a few people still on the energy bridge... priceless...
~Artificer life... Tabbing to select targets, Alt to charge rune arm...periodic combined [ALT][TAB]... og, i am no longer in the game...

This is one of the better ones I've seen. I usually 'forget' to screenie any of my *alledged* mishaps. ;)

Forgetting to switch the Anger boots (for the striding clicky) back to fully unlocked rock boots to boost my run arm DPS for an whole quest and wondering why my shots sucked and I couldn't crit worth a ****.

Forgetting you switched the button you use to push to talk with something you spam regularly and having a rather private conversation with your husband for the amusement of all the guys in the party. :)

I don't know about most embarrassing but,
I ran through half of elite Tear of Dahkaan the other day without any wraps on. How did I not notice that?
On a different note a RL friend was once chatting with me in game about a coming change in guild leadership......accidentally in guild chat. Was quite funny from my screen

I would have to say it was done by someone else.
Gamer's husband came into the room and because she uses Voice over Mic (farking Vadars out there.) the party got to hear all of the... "action" that was going on.
Gamer came back to the group after done, to an ovation. Party didn't forward in the quest much.
This is why Push to Talk should be a learned thing. Save yourself embarrassment, let alone hearing what else is going on in your area.

Yes, I too have done this. I also enjoy not having Feather Fall on during Von2 and similar runs. I'm the guy who suddenly dropped off the radar. :)

I, for some reason, decided to cast Grease on the Red Named Beholder in Von3. Not sure what i was thinking, but for a split second, i really thought it would help.

I've done this with sleet storm. There's no worse feeling than right after you do it...

A couple of semi-embarrassing things I do:
Get stuck bumping into walls, railings, doorways, and NPC's when running through public areas.
Fall into the water while running along the docks in the harbor and have to swim 100' to the nearest ladder to get out.
Click on a warforged toon standing by the AH instead of the AH in the market. Once wouldn't be bad, but I keep clicking (and thus swinging weapon) thinking "Why the heck won't the AH open?".

Oh man I hate that. And every time it's something I can't get away with and the whole group notices. LIke the time I forgot to check them before i went in on my favored soul and couldn't actually raise dead or use my dot. LOL

I'll bite...
First you have to remember intim + shield block used to be an easy way to tank raid bosses like the hound.
It was one of my first times tanking HoX, my intim hotkey was "4" and my teamspeak hotkey was "ALT"... I was spamming intimidate -- occasionally I had to tell my guildies something on teamspeak.
First attempt of the night went horribly, the puppies decharmed quickly and it went south -- someone mentioned that they thought they saw a break enchantment go off.
Second attempt went roughly as well, only this time many people saw the break enchantment go off... the raid group was half pugs, so some blame started going around. People were furious.
Third attempt went fine -- mostly because I realized that my "ALT+4" hotkey was "break enchantment" I told my guildies after the raid and we all had a good laugh, but no one ever suspected the tank.

Swapping mass heal and leap of faith on the hot bar for fast travel to raid site, and then forgetting to swap them back. Puts ya in quite a predicament to say the least. Unfortunately I have first-hand knowledge of this foolish swappery

Or starting an elite Tomb of the Shadow Knight and not noticing you only have 9 thieves tools left.
My friends tend to carry a spare set of tools when they group with my trapper now :)

Being told to go to third base, and going to first base because I was completely not thinking baseball, so I labelled them left to right, 1-2-3.
Finding myself in the middle of a trap and my first instinct is always to swing, because I'm taking damage so something must be hitting me. So there I am, swinging away, dying from the 14 foot blade swooping out of the wall every second.

Twice, in boss fights, have forgotten that Divine Punishment exists. Not that I have it hotkeyed or anything.
In the same place.
On every toon that has it.
In the same place as every other primary DOT spell on other casters.
Also, my Mario skills. They're just an embarrassment.
I also had a barb die to the training dummy when rage expired with 4HP.

Even worse than that... At least twice, during quests, just barely make it to the rest shrine with just a sliver of health left on my barb. Use the rest shrine which unrages AND unmadstones me. *DING* Yeah, dead.... Then laughter follows.
And lets not bring up the Con necklace and swapping for a Cartouche to cast a hjeal scoll... We've all done it. ;)

Already mentioned, but certainly the mistell thing. This actually happened to my husband, but it was just as embarrassing for me. We met online, so we dated through DDO for more than a year before we actually met. And yes, that did involve some very dirty tells, one of which made it to our raid group.
That was something I will never forget.

I keymap individual teleport locations to a hotbar. I made the mistake of placing that hotbar next to another I use for buffs. Midway through a quest I go to refresh a buff and...POOF...I'm in Meridia.
Had to make that mistake a few times before I rearranged my hotbar locations.

I still enjoy when people "push to talk" during:
1. phone calls while playing
2. skype calls while playing (I have done this many times, very embarrassing!)
3. conversations with RL people in the room (I have heard some very amusing conversations over the years !!!)

Helping "educate" others in pre VonN6 party chat, then going to the wrong base myself. I didn't notice the group leader used a reverse numbering scheme from the one in ddowiki.
Not knowing some raids drop BTC gear and looting madstone boots on my cleric. Delayed embarrasment.

I had Break Enchancement bound to 'Control-R' and Heal bound to 'Control-E'. Bad mistake if you hit the wrong one in HoX.

It's the only time I have ever forgot to put my **** boots on.

Typing a message to your party that you *think* is going into the chat window, but instead seeing your screen fill with pop-up windows and then finding (after clearing those windows) that your toon has run off a Lava Cave cliff since the 'R' key was one of the letters in your undelivered message.

I think last night; crucible for the first time ever, but had forewarning on the lfm, need swimmer. Jumnped in with the other guy ahead of me. :D At least I wasn't the first death.

One time we failed to kill the rednames in Shroud part2, during second try, I cast Divine Punishment on a red name, and it died waaay before than the other rednames were prepared. Party leader was angry, saying "who killed it?!".

There really is a trap with having the "push to talk" button.
Ever find yourself pushing it out of habit when you're talking to someone else in the room?

Or have someone else in the room yell something JUST as you click down that talk button?

Unintentionally using Push to Talk while sneezing or coughing. Apparently, everybody needs to hear it. XD
On my monkcher, my Cocoon is hotkeyed to 5 and my Abundant Step is on 6. So I'm standing in front of Lolth taking damage and thinking "I'll heal myself to save the healers some SP". One wrong button later, I get a facefull of Lolth's bellybutton, and then featherfall to my death. Twice. And I still refuse to switch those hotkeys around!

Falling for my stupid guildies undead summons (and repeatedly trying to kill them) while running lowbies through Delaras.
Dropping down the shaft to where the judge/jailer are, and remembering to hit leap of faith so I have feather fall. Only to have it active a second too late....it did however cut down on the whole "pre-fight conversation"

New one for me yesterday. Lag spike in the 12. Fall off bridge. Into lava. Died and I wasn't even on a quest. Where was my resloc? Back in the Bugbear Inn in Tangleroot. lol

makes a mad run for it, gets to the quest entrance...enters the quest incapacitated.

Running through the deadliest traps or trying to jump to distant locations...
...and then realizing that I'm not playing one of my many Monks...
I've speared, chopped, incinerated and splatted my non-Monks so often that the Looney Tunes Union (Wile E. Coyote, President) has sent me an employment offer letter.
