Hi guys /gals
I am conducting a research about online friendships among MMO players. The research is a part of a graduation paper in Psychology.
I mainly interested in adults who were diagnosed with learning disabilities as children, but I will be happy if people who were not diagnosed with learning disabilities will answer also for my control group.
the survey has three parts,
first part you have to rate from 1 to 5 twenty (20) statements about real life behavior.
second part you have to rate from 1 to 5 thirty (30) statements about online friendship.
third part is a demographic survey with twelve standard questions.
the survey is anonymous
No one will be able to identify you or your answers, and no one will know whether or not you participated in the study.

EDIT: changed the survey to shorter version (removed approx 25% of the statements) with more explanations for each part.

I took your survey. You really need to consider making this more professional if you want people to take it seriously. The entire 2nd page asked the same question about 10 times, simply in a different order. If you learned this is appropriate in some class, ignore the teacher. People are not stupid and they tune into this immediately. After seeing it 10 times, it sounded patronizing.

I'm sorry but I agree with this assessment, OP.
I am also tempted to give you wrong/inaccurate information because of the following reasons:
+ The survey is too long, I want to help but I feel bored and I just want to get over it.
+ Some questions are double barelled/vague: "online friend or friends" Not sure how to channel, figure out a reference.
+ I (might be) severely uncomfortable disclosing personal information, particularly about my medical status
+ Some are binary questions, rather than scalar questions.
Beyond the above issues, I strongly suggest you reconsider your approach to have research design specifically targeting people with disabilities. For instance, how can a person with a known reading disability comfortably answer repeating questions that just get structurally transposed?
Regardless, thank you for taking the time and effort to do all of this. I really hope you succeed and learn from the whole experience. :)

Try a real life test...
Take two samples... 1000 DDO players and 1000 WoW players. send each group to the opposite game. See who has the hardest time learning :D

I have added more information and explanations in the OP and in the survey.
also tried to remove somewhat redundant questions, the new questionnaire have 25% statements less.

It gets old.
It does, but do you realize why the question gets asked over and over in real life surveys? To be certain the person is answering honestly vs. haphazardly screwing up the data points.

Eg: Do you solemnly swear to tell the truth, the whole truth and nothing but the truth?

There's clever ways to do this without having to ask questions over and over again. Good survey design can include a number of traps including:
+ Asking the age of the person (or any other slightly personal information) at the beginning, and then asking it again at the end;
+ Detecting straightline answers
+ Reversing the statement structure (so that all questions are not affirmative questions)
+ ... and other more interesting survey types (hot/heat maps, classification, Top 3)
I'm just glad the OP changed some things in the survey.

This question is confusing.
Do you mean in game or IRL?
The whole third page could use a little more clarification of the instructions.

this question meant to be in general.
as you have a separate question that ask about people in the MMO and outside the MMO.
