From the last Lammania Update release notes:
NEW - Tome Upgrades can now be found in random treasure. These wondrous items will allow you to upgrade an existing inherent bonus in an ability score to the next higher inherent bonus. Upgrades that turn a +4 Inherent Bonus into a +5 Inherent Bonus will be able to be found in treasure levels of 27 and higher.
So...that's their response to complaints +5 tomes were too rare at .00001% in EE CitW chests. Huh, interesting

I like it, IF it works like one would think it works and it's a drop that you can get just like +1/+2/+3 tomes right now. If it's actually on the level 27 loot tables then I can see Turbine selling a ton of +2 loot gems. I'll have to figure out where/if the +3 to +4 tome upgrade drops, then work on upgrading to +5's. Overall, this could be some new chase loot to look for or it might be so rare there's no point at all, like 1 in 10,000 odds or something like that.

Well, how many +4 tomes have you looted from chests?

Yes it might be like the drop rate of yellow dopant and iv still yet to get one even with the so called increase in drop rate.

It's not completely clear from the release note (surprising, huh?) if the only upgrade will be the +4 to +5 version. It states that one will drop in level 27+ loot tables.
Another assumption of mine is that it will drop at a rate similar to tomes themselves, but it could also be a hyper rare store item drop. Store items don't typically have such a very specific and noted chest level range though. Remember that fate tomes drop as early as Korthos. This fact makes me think it's more standard like +1 through +3 tomes.
I think that a +3 to +4 tome upgrade item would drop in level 26 chests, but as it's not specifically stated, who knows. If you can get that upgrade then finding the actual +4 tomes lose a bit of their relative rarity.

Well, it is clear for me. All tome upgrades will be dropping in the chests with the +4 to +5 version dropping only at certain chest level.
Yes, I believe they may be dropping with a similar droprate to the tomes. So +1 to +2 will drop with about the same frequency as +2 tomes (maybe a little lower) and +4 to +5 upgrade will drop as often as +5 tomes. You're still excited? :P
But I agree that for the time being it's all just a guessing game.

Am I the only one reading it as blank step up. Rather than just a +4 to + 5 it sounds like it could be a +1 and make it +2 or a +3 to a +4 etc could this be the new tomes on offer in the store ?

Yeah, I read it as an upgrade to an existing tome too. That actualy makes sense from a game design POV since now you can collect your way to +5 instead of just waiting for lightning to strike.

Description of chest-buffing ability says you can't go higher than lvl 25 I think, but my bard has no such feats, so i don't know for sure.
EE Deal and the Demon - I bet devs know we all farm that first chest.:)
Not every chest has the same level as the quest it is located in (see DQ1 side chest at end).
Medium jewel of fortunes can get from collectables (large eberron shards).
