One of the guardian doors wont open even though the guardian is dead, the plate does nothing except light up.

Which guardian? I ran it a bunch the sneaky way 2-3 weeks ago on my bard. Screwed up on the last guy and had to range him down, but it all worked out in the end (till the rest of the party tried to follow and died horribly in red alert).

Not sure if this matters, but did you leave Iona's staff behind? I've done that before, and had to run back for it.

Interesting, I noticed the exact opposite. I ran the quest last night on epic and the only door I needed the staff for was the very first door. Every other door either opened when the guardian was killed or didn't require opening.
I, also, noticed the fire trap in the room prior to the Giant Skelly boss is no longer there. It has been a while since I have run that quest, but it looks to me like they nerfed it completely. This quest has certainly changed.

You only need the staff after the first door if you are doing the sneaky method - if you are just killing then you can leave it after that. But if you are sneaking you need to use it at each door. As the OP said the guardian was dead then it should open - possibly not dead but buried himself and hiding?
That said I make a habit of picking it back up again - just in case....

That trap is there on EE. Run it a bunch on EH, then run it EE and forget it's there. :)
As is the trap in the initial dropdown, which is gone on Hard. Usually you just kill the Giant coming up, but he sets off the trap by pulling the lever in the room you drop down into (by the mirror). There is no lever in EH and no trapbox for the xp's.

Scorpions have been being shy lately.
Couldn't find the last one in haverdasher yesterday, think he burrowed into a wall.
If he was actually dead, than it should have opened. Either way sounds like a bug.
