1. "You are not facing XXX."
2. "XXX is not in range."
Having it work this way is probably an artifact of a testing environment with 0 ping, aka in a LAN. This is not how a game is played however. Facing checks should always be done locally, there is absolutely no excuse not to...

1. Lag caused by varying UMD score.
2. Latency between using wands/scrolls/clickies and their effects.
3. This bug. (http://forums.ddo.com/showthread.php?p=4650707#post4650707)

Don't know if Ertay is right about the reason, but it sounds plausible. This is annoying as hell for a caster and according to some of my friends who I count as experts, it's been getting worse since U14.
These'd be my top even though I don't just play casters.

There's a lot of bugs and a lot of severe bugs, but none are to me as obvious and infuriating as the ladder bug. Every time I try to climb a ladder and start warping back and forth on it, I see red almost instantly. It's robbing me of control of my own character and has gotten me killed repeatedly.
Second one, drow priestesses sacrificing mobs while paralyzed or encased in ice. That's not how it works, Turbine! It's like letting players heal while stunned.

1. Ladder bug would be my choice too. It is one of the ancient ones and it's affecting every character.
2. The game started to freeze after update 14, what's up with that?
3. "The Ruins of Gianthold is blocked by Gianthold"

1. Destinies disappearing during LR
2. Tomes disappearing during LR
3. The new bug making quests impossible to complete
4. Stunned mobs moving/taking actions
5. The DDO store bag bug from the first wave of sold bags
6. CitW as a whole. Much of it is now WAI, but that doesn't make it any more tolerable.
7. Smites and many other abilities not producing offhand attacks while unarmed
8. Warforged healing amp
9. All incorrect item/ability descriptions. This is inexcusable especially after months/years
10. UMD lag

The only two that are annoying me most are:
1. Quests (or options within quests) that can't be completed for one bug or another.
and
2. Judgment ... please it's not funny anymore.

Not to mention the heresy of complaining about the fact that we only have such a farce of a testserver... No working charcopy tool, no way to get xp/unlock class abilities, no gear, no nothing. It's more like a alibi server so they can say they didn't see those bugs in testing so they are unpredictable.
This and "release notes". If I were tasked with actively destroying any faith left in the development process, I couldn't do it any better.

LOL, well maybe they should just turn it off for a year or 2 like they did before.... Or they could just pretend nobody ever reports anything like they did before that. Remember when 100's of people would report specific bugs, and then Turbine would come in and say "What? We've never heard of that before":D
But yes, I would agree..... The bug reporting tool or page should be something that works, even if it feeds solely and directly to some system that never gets turned on let alone used. :o
It's really hard to comprehend how how it could be so consistantly and universally broken for what 4 1/2 years now.... Since at least the Reavers Reach release......

The fact that the person in charge of DDO at Turbine (Producer, Director, etc) still has a job. Fix the glitch. Start from the top and go down.
Overwhelming Critical.
Obtaining this feat is not a simple task for a barbarian and requires considerable thought with regards to your build because of limited feat slots. I'd greatly appreciate if it did what it was supposed to do. Because their PRE boosts crits on 19-20, Barbs don't benefit nearly as much from an extra crit threat as they would from an extra multiplier on 19-20, and that is especially true for greataxe builds.

Some of these things may not be things that are "bugs" but they do annoy me and I see almost no benefit.
1. The lag when switching characters.
2. UMD lag.
3. Mobs still getting attacks off just after becoming helpless (though it is funny to see a huge damage guard proc when an opponent becomes stunned, and then they take 450 damage instead of 300...)
4. The random freezing that's new with u14.
5. The floor being out of range.
6. The way certain words are filtered out when they clearly shouldn't be, "Con" and "(spell) Penetration" come to mind, but there are a lot of other inoffensive and harmless words, like "dummy"
7. The way other people can change my lfm to public. At least there's a notification when it happens, or I would be real irritated.
8. Some actions can have an "out of range/blocked" notification, when, if they were in range and in line of sight, they would target the user anyway. Negative Energy Burst is the one that comes to my mind, I think the cleric's radiant burst is this way too.
I'm a bit of a bard lover, so I'll put these last 2 for them :D
9. All the bard songs that have been bugged basically since they've existed. Includes, spellsong vigor and sustaining song failing to work against warforged, Ironskin Chant and Spellsong Vigor not working on pale masters (recently I've noticed that now getting into form also dispells them...devs must have taken a crack at this bug and botched it worse or something...) Enthrallment not allowing suggestion songs, some of the offensive songs don't work on orange names. A minor one that I think is a recent bug, and I only pvp when I'm truly bored, Fascinate is hitting me in pvp, when I'm the one playing it. I haven't tried it in PvE since u15, so I'm not sure if it's happening everywhere or not.
10. And just to have a #10 that's related to bards but not (quite) their songs, the Anthem effect on Blasting Chime has failed to work at all since it was released in u12.

The engine and all the little "hacks" that have been done throughout the years to make the old code that can be inferred as not well documented work with "new code".
Case in point: UMD should not be referencing anything or doing anything unless it is actively being called upon. Call by need. There is no need to do any queries unless something is actively being tested and then and only then that object. Are you really going to unequip RR or alignment based gear when UMD dips (but not actually to the threshhold where it might be a concern) each and every time? Or is testing each time you load into a new instance/map more than sufficient. People who take UMD dont do such for the weapons....they do it for access to spells.
Efficient use of data structures and algorithms. Honestly, if other games can do the same base things that DDO does but their main issue typically is "gameplay balance" as oppose to bugs (some bugs have existed in DDO for years) on the forums, and break less per update, there is a problem.
Is DDO a bad game?
No, but it is far from its potential. In its current state I cannot in good faith recommend it to friends that are used to high quality MMORPGs. There are too many bugs to the point of not knowing if things are intentional or bugged if you are new.
Will I continue to play it?
Yes but only until something offers active combat that does not revolve around a gamepad, unless something changes. Such things are coming down the pipeline. Also of note, Wizards isnt in control of the only d20 system.

Bug or not, switching between weapons or equipping a shield freezes everything up for a few seconds.
ALSO, Lay on Hands isn't instant. When I'm using Lay on Hands, I need it then, not a second later when I'm dead. :(

Completely selfish bugs that annoy me (the not facing XXX is annoying as heck, or spells just not working at certain ranged... but that doesn't affect my main so i they don't make my list XD). In no particular order:
1) Potions/clickies that don't activate properly. Clickies are annoying, but potions not working when i click them can be life threatening.
2) Weapon swapping not working, ESPECIALLY ranged to melee.
3) Any feats/abilities/powers that are supposed to work with Ranged Combat but do not (Fabricators Bracers, Improved Martial Arts, and maybe Combat archery - i need conformation on Combat Archery)
4) The excessively long delay between Stunning Fist activation and its actual effects (In quests with easy enough enemies, i will sometimes kill one or two enemies before the stun even goes off D:!)
5) The "Climb" Animation interrupting any and all actions, but still using their "cost". Sometimes this is annoying, such as when losing a clicky or spell. Other times its infuriating, like when losing a SP potion or putting a looong recharge time ability on cooldown. And sometimes, it can ruin a quest, like when using a quest specific object which has limited uses, or a kobold teleporter.

1. UMD lag
2. Character swap lag
3. Destiny Xp/tomes disappearing on TR/LR
4. Bank eating things...expecially bags.
5. Item swap failure
6. Assassinate bugs
Those are the biggies. After that I would go for broken items and item effects (like my eMidnight Greetings giving 0 to-hit bonus, handwraps). Then cosmetic stuff like tearing, string table error names, etc.

Any other invasive species.

I would vote:
1) Fix the development/Quality Assurance process. Something is amiss since the planning of U14. The quality has dropped significantly.
It doesn't matter what is fixed now since there will be more broken things if the process is buggy, agreed?
