This weekend I put up an LFM for Cabal for One and Feast or Famine on Hard. The first person to join was a level 16 sorcerer with 88 hit points.
I asked him if he was new and he said he was. I asked if he wanted a little help and he said he did. I took down the LFM and took him into the GH explorer area, where he was one-shotted in about three seconds. I explained that explorer areas are easier than hard quests and asked him about his CON score, his gear, etc… He was very open to suggestions and seemed to be genuinely interested in learning more about how to improve his character in this great game.
He said he had a CON score of 8 (he was an elf and had actually put 2 points into CON, so he thought he was okay.) He had on an Improved False Life belt for 20 extra hp. He had some other almost useless gear, and a bunch of completely useless gear.
He had started playing DDO very recently, bought Veteran status to start at level four, and then someone used a Stone of XP on him to get him to level 16. He was, admittedly, clueless.
However, he was very willing to listen and appreciative of the time I spent explaining stuff to him (which was only about 15-20 minutes, so it was hardly a big deal.)
If anyone else is level sixteen and is having a hard time, feel free to look up Praetoreus, Jarladdin, Archenpaul, or Gregorovic on Thelanis. I’d be happy to help you learn how to survive and enjoy this terrific game.

The good news is that he can use a free LR to get his stats correct.

I've been playing since 2009. I recently purchased Vale, and it is now the highest-level pack I own. (GH was the old highest.)
I've been f2p with the exception of adv packs since I started playing. The highest level toon I've ever had is my current level 17 monk/rogue. I used a stone on him, but he was level 11. I had a level 16 Ranger before that, which I leveled the old-fashion way of grinding XP till my eyes bleed.
I can honestly say that even though I've got years of playing experience, and been through countless adventures, I still have a somewhat low level of knowledge about this game.
I know a huge factor in toon survivability and combat prowess is gear, but that's as far as I know. What gear? Where do I find it? Which item is best for a specific class? I've skimmed through the wiki a number of times and still have no more a clue about what to do than I did before. I feel as though I'm doomed to entrust my survival to random +5 Armors of Something Useless, Bracers of +7 Skill I Don't Use, and basic +6 Ability Enhancement items.
Oh, and being that I've never leveled to 20, I have never been more than a "first life build."
Granted, I like to build toons that I find fun to play. My level 17 Monk/Rogue was a Ninja Spy/Assassin (recently respec'd to Spy/Acrobat), and his main mechanic is his sneak attacks. Sure, when I add my +10 Dex modifier to my sneak attack with staves, I'm doing nearly 60 extra damage. But that's useless against undead, fortified enemies, bosses, and when I'm generating threat/soloing. But I like the RP aspect of it. Creeping through the shadows and striking an enemy down in one hit is pretty awesome (however useless when in a party as every seems to rush for kills first).
But I'm sure I can still outfit such toons with helpful gear? There has to be SOME hope. I know the Vale offers some of the best loot in the game, but when I go there I feel like I'm trying to read a book that's written in a language I don't understand.
I dunno, maybe I'm hopeless. Destined to forever wear basic random loot.

Are you on Thelanis?
I think I have a level 3 there. But my main servers are Argonessen and Cannith.
Cannith toons:
Aleraith - 7 Monk/10 Rogue
Drurask - 1 Fighter/5 Wizard
Haelwynn - 4 Ranger
Donnegall - 4 Rogue
Argonessen toons:
Orgaana - 16 Cleric
Helvetie - 8 Ranger
Donegall - 4 Rogue

You can try throwing Precision into the build. Precision should be allowing (http://forums.ddo.com/showpost.php?p=4549200&postcount=5) you to bypass 25% of an enemy's Fortification.
You might also try your luck with Improved Sunder. Improved Sunder is going to be dependent to a degree on your Strength since it requires the enemy to fail a Fortitude save (DC 14 + Strength modifier). Though regardless of whether the enemy fails or saves, the enemy's Fort save is reduced by 3 per stack (max of 5 stacks). So chances are decent it will land eventually with a modest Strength score.
Throw in the Opportunist rogue feat and you'll see the ability to potentially bypass 45% of an enemy's Fortification.

Precision is a good choice but the Opportunist part I'd use elsewhere. I have Improved Evasion, as this build is going to be 7 Monk/13 Rogue. And I believe that also means I miss out on the second Opportunist feat.
Anyway, it just seems that would be a waste since Fortification only deals with a fraction of the times my sneak attack doesn't work. At this point though, I'm ready to just cap and TR. The build didn't turn out as I expected, and in the long run, building for fun actually makes the game less fun. 41 AC and 330 HP is a bad combination in elite Vale quests.

Some one that has purchased the MoTU pre-order package and is level 16 or higher has to use one on you.
Try asking in general chat in Harbor area.
Most people will be happy to hit you with one.

There's an NPC in the harbor (near the Market exit) who passes out the stones. You are only eligible to recieve stones if you have purchased the expansion (Menace of the Underdark) and the character that is speaking to the NPC is level 16 or higher.

nice, a rare occasion where someone with a clue meets someone clueless who is clueless because this game does a very good job in confusing new people and not because he chose to be a stubborn moron :) I like it.

just wanted to say that i am willing to help too, in fact i usually try to find newbies in the harbor and gear them up. so if anyone needs some guidance, contact me in Thelanis on Tanchen, Chuzo, Zerreff, or Gruno (gonna make Gruno in a day or two) send a letter even, if need be.

I know what Turbine's thought process was behind these stones yet they may end up driving more players away from the game than they will sell MOTU expansions.

My advice is to read the forums for your class. I always find great tips about what to do with my characters there. Everything from gear to progression. And if you search and don't find what you're looking for, you can make a post.
Tip for searching the forums: after you use the forum search try Google (something like "DDO PM gear"). The forum search isn't great, but Google often finds the threads.

OP
You sir; are awesome.
+1 Rep for showing what this game is truly all about.
It is all to easy to cry "Noobsauce!" and drop someone for
having an inferior character but it shows great integrity to take to time
And teach someone how to do it right. And help them
To learn to play and better enjoy the game.
I tip my hat to you sir, well done.

There is a Dwarf by the entrance to the Harbor.
Speak to him and he will give you the stone if your character is over Lvl 16
As an earlier poster has mentioned, you must have purchaced the expansion and not have gotten the
Stones on another character.
