No. It doesn't apply to raids. They list the specific quest chains and quests that it applies to.

I think it's only for quests/quest chains, not for raids.

Things like Gloves of the claw in Claw of Vulkoor end reward list and similar.

Unless stated otherwise. There are quests that will drop rare items from the third list. Dreaming Dark for instance will drop an Ioun stone every third completion (tested and confirmed on Lamannia).
