I'd recommend:

Delera's Tomb: (levels 5 to 9ish)
- Good low level xp farming quests
- Voice of the Master trinket for a 5% xp boost from all content levels 5 to 19

Demon Sands: (levels 10 to 13ish)
- Lots of good xp
- Some nice loot

Then you have Gianthold, Vale, and Dreaming Dark for most of the rest.

I'd also recommend picking up The Devils of Shavarath to help out the xp on the last couple of levels and there are also great items to get there as well.
