ML 5 ioun stones drop in lv 5 quests, ML 9 ioun stones drop in lv 9 quests.
there is no specific quest that drops them often, though i suppose ones with more chests will net a higher chance.
i wouldnt bank on actually getting anything though, even if you ransack with 4 characters weekly.

Not as cut and dry as that.
I pulled a +2 Str stone with Suppressed power as end reward from Vault of Night recently.
There is a "special" Loot list that has a chance of appearing. You can get say, bottle of air, or full plate of the deep from that list, as well as ioun stones.
it seems to disappear once the quests become high enough in level though. Not sure on the specifics.

I have no idea how many low level characters I have played, serious case of altitis, and have never seen a single stone in probably thousands of chests. the drop rate must be something like 1/10000 of 1% :)

I got mine as an end reward for an Elite Irestone run.

There is one quest I know that drops the Protection stone ("Dusty Rose" I think) often enough for me to say that it is the quest I would run to farm for that specific stone; however, with that said, I am not saying which quest. :)

I pulled a dark blue from Butchers, and a dusty rose from ....sunken sewer?....the real quick one with the trog at the end, swim and climb the ladder to get to him.
