Currently, auto-attack only works when I have a target hard targeted. I would like a way to simply have my character continue swinging regardless of targeting status.

Keep your finger down on the left mouse button.
/not signed, as the thought of a group of melees windmilling their way through an entire quest turns my stomach.

If we have a button to run ceaselessly when we could just as easily tell people to hold down the Up arrow key, I think my request stands to reason.

i'll be happy if they fix auto-attack to actually work since half the time when I do have it on, my barbarian just stares the training dummy down instead of swinging at it.

There are times it needs a jump start. Most of the time I'll have autoattack on while running part 1 of the Shroud. I'll click on the portals or tab to next target and I won't start swinging until I hold down the mouse. After that it works fine until the next portal.
I have a similar issue with the ship dummy. I'll have autoattack on, but I'll need to click it again to actually start swinging. It's reporting a false positive.

There's actually a combination of buttons you can do to 'glitch' yourself into permanently swinging until you roll or something else.. Not like I can remember it, just thought I'd give input.

Here is how you do it.
1. DDO game client MUST be in "windowed" mode (no full screen)
2. Hold down your mouse button that makes you attack
3. While holding down the mouse button, drag your mouse off of the client to your computer's desktop
4. Click and hold down the other mouse button so both are being pressed at the same time
5. Let up on the first mouse button (the one you use to swing your weapon)
6. Let up on your other mouse button
And there you go! You can now go back into the game client and you will continue to swing your weapon. The only thing that will make you stop swinging your weapon is if you press the mouse button that makes you attack (in-game).

I daresay the reason turbine doesnt have this feature working the way you want it is because of the fiasco with unattended combat macros in AC1.
Also, dual boxing...simply run two toons with auto-attack on through a dungeon, a healing hireling in tow for both, let them crunch whatever gets in front of them. Obviously is wont work on EVERY quest, but enough to end up with alot of the problems that UCMs caused in AC1, even allowing for the fact most dungeons/etc are instanced in DDO.
