My understanding is that if you Lesser Reincarnate, you become level 1 character and must speak with a trainer to get back to your original level (12 in my case). Do you end up with only three inventory tabs, or do you keep all of them?

You get an option to change your appearance. I am quite satisfied with my toon's appearance; does it remain by default, or do I have to go through the same procedure of toggling hair, nose, etc. which I did when first created him?

Do I have to pick all starting feats, or does it work same way as when you start a new toon -- the game gives you bunch of standard feats, and you only select one or two depending on class (Bard in my case)?

IIRC you keep your inventory tabs as well as all your gear (though I expect most will be red-outlined as too high a level to use) and you will need to reset your appearence. It is just like starting and leveling a toon with the exception that you get your XP for free at each level and can relevel immediately until you get to where you were. Also, any tomes you have eaten will be reapplied when you reach the appropriate level rather than all at once at the beginning or end.

That, and you can only change classes based on the type of lesser heart you are using.

You _have_ to select a new appearance.
You keep everything else, your bank slots, your favor, your inventory bags, and your exp and even the effect of the read tomes (appropiate for the level, if you read a +4 tome you get +1 on level 4, +2 on 7, etc)
You start the Char on level 1 and have to advance until you hit your old level, iir the only way to change skills is to lr or gr.
