I find my monk hitting enemies until the last few hits, not missing but a few hits, then as the enemy gets closer to dying the monk simply misses every shot.
Additionally the cooldowns and combinations of the monks enhancements and feats are very glitchy. Auto-attack can disrupt meditations by simply looking at a collectible. When the cooldown of the meditation and wholeness of body are done they are still not usable for a few seconds. Yes seconds. Plural.
Also, much like a critical chance percentage and other percentage based numbers there are many streaks of success and failure. Often any class of champion can struggle or exceed based on these percentages that leave random streaks.
Finally like many other bugs and issues in DDO, it is not likely that any of this will be addressed. Continue with the mediocre job Turbine. That is something that is very reliable.

Can't say that I've noticed any of that. There's always going to be the percentage chance that you miss things and, yes, they can come in annoyingly long strings. I think I missed 8 stuns in a ROW the other night just because of how the rolls came out. Think of it this way: you're hitting all the rest of the time. If you need more too hit, rework the character / items until you're missing only on a one. I mean, if the monk is missing every shot as the mob approaches death, nothing would ever be dead.
The finishers can be a little finicky. They always have been and there's a large number of things stop a finisher. I don't play with auto-attack on very often, but I've never noticed that moving the mouse off the target changes anything. Have you tried hard-selecting the target to make sure that nothing else steals focus?
As to the last point, I've never noticed that either. Once a timer comes off of cooldown (and monks have lots of cooldowns), I've always been able to hit it without difficulty unless there was a mis-registered click (common in when lots of buttons are being pressed in the heat of battle). But, as meditation and WoB are used outside of battle, they've always worked for me. I've had no issues with immediately clicking WoB as soon as the cooldown is off. I'll check again when I'm on to make sure I'm not mis-remembering.
I don't know how you play but you might try moving to a WASD or ESDF scheme. Monks almost beg for that kind of play though I acknowledge there are many monks who don't who are superior players. Just about all my monk(y) abilities are hot-keyed as opposed to mouse-clicked and it works wonderfully.

Agreed with the post above. I've been playing a Monk for about 6 months now and I've never seen this issue before. You can tell what you rolled when you 'miss' by looking at the dice icon.
Properly built melee's should only be missing when they roll a 1(except for extreme content). So next time you miss, look and see what you rolled(this may mean that you need to turn PA off, or use better handwraps, or as the poster above said. . .redo your whole toon).

This. There are many times you may need to turn PA off if you are missing on more than 1s and 2s. I find myself needing to do this a decent amount if I'm running dungeons on elite rather than normal. Also check better wraps could be the key. If you are missing a lot try to get ones with a higher bonus. +5s will help you hit much more than +1s. Also if you are fighting particular types of mobs a lot try to get greater bane wraps for those types. They help a lot.
