Just wanted to fill everyone in on my experience.

I just Tr'd for the fifth life. I went from barbarian to monk. Stunning fist was NOT on my list of availanle feat choices. I created/skipped korthos, and bee-lined it to Lockania for my free feat eschange token. After speaking with Fred and trying to exchange either feat ( although stunning fist is a class feat it should only show when i choose to exchange the class feat) still no stunning fist.

I submitted a ticket and was referred here.

Anyone find anything else?

Without selecting a feat on the left side (leave the box empty) , select the feat you want from the right side then choose the feat you want to replace.

That has worked for other stuff but I dont know whats up with your case.

I'm not sure, but I think Stunning Fist is only available to be taken as a Monk bonus feat.

Stunning fist was removed from the game as a regular feat because turbine likes to do random stuff while coding the game for fun just to see what will happen. (There was no reason to do, the feat always had a prereq of "Class: Monk")
It also cannot be swaped in via fred.

You can only take it while leveling up, and only as a monk bonus feat.

stunning fist has been mentioned as a known issue for not working as a feat exchange. It has to be taken while leveling during one of your monk levels. I have a clonk that had the same problem. Welcome to ddo.

it will show up but only at the levels that you get your bonus monk feat and may only be taken as such and if you go to exchange it it must replace a feat take at that spot

Actually, you swap it in by use of Fred. Or at least you could slightly prior to update 7. I swapped my level 17 Dark side monk at that time to a shintoa because my general lack of wisdom wasn't really the best for the newfound save DC on ToD.

You just need to make sure you are swapping out one of the feats you took at a monk bonus level, because otherwise it won't show it in the available feats to be swapped in.

-edit: seems to have broken somewhere in update 7 or 8 then, it used to be possible though.

Known issues list (http://forums.ddo.com/showthread.php?t=150534):

NEW - It is currently not possible for Monks to obtain the Stunning Fist feat by speaking with Fred the Mindflayer. As a work around, it is possible to select Stunning Fist during a reincarnation.

yeah i swapped it in to my experimental helf clonk (the one you laugh at Axer with 10 con...) took quite a bit of stuffing around. that was about 2 weeks ago.
