Specifically about Vale of Twilight, but may have relevance to other zones. Does the timer begin the moment you speak to Gustavine Kyerrh at the Twelve, or when you actually enter Vale?

The guest passes start their countdown the moment they are used. Doesn't matter if you spend an hour in the Harbor - the timer is still running down.

So what's the definition of "used"? Does speaking with Gustavine Kyerrh activate the pass? If not, what does?

I guess my real question is: how exactly do I use the pass?

You use a guest pass by purchasing it from the ddo store, then casting it on another player. You cannot use them on yourself; they only work on someone else.

You have to drag it out of your inventory and onto a hotbar, then select the person to whom you want to have the pass. Then, with them still selected, you click on the pass in your hotbar.

I believe the timer starts as soon as it hits them...

If YOU want to get into the Vale, you have to purchase the adventure pack, or have someone cast a pass on you. :)

Steps to using a guest pass:
1.) Purchase pass from DDO store. It will appear in your inventory. (Note: you must own the pack to purchase the pass)

2.) Place pass in hotbar

3.) Target recipient

4.) Click on pass in hotbar

At that point they can act like they own the pack and the timer begins, whether they're using the content or not. I think (but am not certain) that time spent logged off does not count against the timer. I am also pretty sure that you can finish the quest after the pass has expired, but you cannot enter any other areas provided by that pack.

The guest pass does not start as soon as someone gives it you... it only starts as soon as you enter a quest or explorer area from that pack. Once it is started however it counts down no matter where you are or what you are doing.

Example:
If you get a guest pass to the Vale of Twilight your timer would only start as soon as you enter the Vale or Subterranean explorer areas.
If you get a guest pass to The Carnival in House P the guest pass only begins when you actually enter one of the quests.

When did that change?

Back when guest passes were first created, I tested them and the buff timer didn't start running until you zoned into an instance that you couldn't have reached without the guest pass.

To make it clear: someone GAVE me a guest pass days ago. I had not used it; here is a screenshot from today:

See the scroll icon in upper right corner? It appeared when Serae gave me the pass, and it has not changed for days. I had not tried to enter the Vale yet. What do I do to enter it?

Drag the guest pass from your inventory on an action bar - target yourself and "cast" it.
You should now be able to enter the vale.

That's what I was looking for. It is pretty obvious to me that countdown did not yet start. OTOH, I noticed that Gustavine Kyerrh still shows a coin icon over his head, so I was wondering if I need to do anything with the pass before speaking with him.

I went in last night, so would like to clarify some misconceptions:

1. There is no "pass" in my inventory; nothing was added to it when Serae gave me the pass, just the icon in upper right screen corner.
2. Even though Gustavine Kyerrh had a coin glyph over his head, he spoke to me and gave me the "mini-quest" to meet Aaseamah Screven. Nothing happened with my pass icon, so I suspect he speaks to everybody level 12+ whether or not they have Vale pack or pass.
3. When I found the gate to Vale of Twilight, I did not have to click on the icon or anything else. I just clicked the gate and entered. Message appeared "You pass is now active", and 90 minute countdown began.
4. When I recalled from Vale and teleported to Harbor, countdown continued. To minimize time lost, I had my alt shop for Soondar's replacement arrows :)
