I've noticed the TWF feat chain in a lot of monk builds. Do these affect unarmed attacks or is it for circumstantial kama use?
I have TWF on my newbie monk but it doesn't show a second attack the way dual wielding would so I wasn't sure of the effect.

It is for unarmed and kama combat. The TWF description (http://compendium.ddo.com/wiki/Feat:Two_Weapon_Fighting) says (red to highlight):
Reduces the penalty for fighting with two weapons, and increases the chance to produce off hand attacks when fighting with two weapons (or as an unarmed monk) by 20% to 40%.

You will notice it more and more as you level up your monk, most monks go unarmed due to fists being significantly better than a monks other weapon choices. Level 20 monk fists do 2d10 for each fist and they get full strength damage for both fists where as other TWF melee classes get half strength in the off hand.
FYI, Two blocking and defense do NOT work for monks unarmed combat.
