Let's discuss gearing a monk...
First we need to find out what we need to have, then we can find the optimal gear to fill those needs. Some bonuses are limited by slot (+concentration for example has few items), while others can be on just about anything (rings). My goal here is to find optimal equipment, randomly generated, named or even sets that will work well for a monk.
Things we'll need:
100% Fortification, Greater False Life, +6 STR, +6 DEX, +6 CON, +6WIS
Things we'll want:
Exceptional STR, DEX, CON, WIS, +X Concentration, +X Balance, +X Jump, +X Protection, +X Resistance, + Natural Protection, +Dodge, Feather Falling, +To-Hit Bonus, Armor Bonus +X, X% Striding, +X Move Silently
(Feel free to add more, such as Disease Immunity, Blindness Immunity, etc)
Let's start off with a couple slots that are "easy" to fill, or at least cover some of the basic foundation.
Icy Rainment, Dodge Bonus +4, Protection +4, Resistance +3
Minos Legens, Toughness (20 HP), 100% Fortification
Greensteel Cloak +10, +15, +20 HP, DEX, +45 HP, Displacement, Blur
I'm open to suggestions, additions, etc.

Yes, a greensteel cloak with 45HP is ideal for that slot.
Looking through the possibilities of cloaks, there's little to choose from in specific regards to enhancing a monks life expectancy or DPS. There are some nice little bonuses, such as Cloak of Shadows (+7 hide, move silently), Cape of the Roc (feather falling), or Cloak of the Zephyr (blur), but really nothing that directly contributes to survivability or DPS.

if you want blur effect the tier 3 can grant a blur permanent effect
i choose to make green steel cloak all earth because i like a pet and i like the grab effect!it is also nice the positive effect...1 clickie of true resurrection is very good!

1st off theree are somethings on your list that monks DO NOT need; featherfall and disease immunity. There are others that will greatly lose there usefullness as you level; jump skill (a clickie will help here if you really need it), concentration(it should be high enough,especially with the capstone it wont help much). As for stuff you will want or is "essential" monk gear:
head: minos or minII helm
neck: monk necklace, this will provide your +6 con, ****ao can provide +15 concentration
trinket: bloodstone, shimmering arrow, litany if neutral or even the new upgraded spell absorbing ioun stone
cloak:GS smoke, cloak of zyphyr
belt: one of the Barbarian belts that have GFL or epic belt of the Moronan
ring1: make one a str slot which can eventually be filled with a ToD ring, Barbarian sets are a good choice
gloves: spectral gloves, spectral gloves, or spectral gloves, eventually want to try to epic them
Boots: lots of options here and will probably be swaped out situationally, striding, firestorm greaves
ring 2: wisdom slot that will eventually be a ToD ring, the monk rings have +6 wis and +1 ex other stat by set
bracers: chaosguard, +8 armor bracers or Jid'z tet'ka monk bracers, eventually epiced for +1 ex wis
armor: icy rainments or DT w/ +5 resist/+5 prot/+4 insight(ideally, gl on the grind)
goggles:tharne's
exceptional stats can be put on epic items, unfortunatly the only real viable placement for monks:/ as your ToD ring slots should have Holy/shock/acid ect to help with DPS

I don't think monks need featherfall, Slowfall works. I never had an instance leveling up to 20 where I needed a featherfall item.
Other items, Bubble belt, great for swapping out belt for waterbreathing and not an important slot to loose.
I went for a second GS item Helm with Heavy fort, 45hp, +6 exceptional concentration(con skills) and Protection +5 stoneskin clicky.
Cloak was GS Blindness/disease immunity, imp regen +6wis +10 haggle/diplomacy and Deathblock
+6 Con belt from Shavarath quests, I perfer greater false life belt
Spectral gloves/Epic Spectral gloves
Shintao and/or ormi's necklass
2ToD Rings with burst damage (holey and shock my prefrance)
Jedz-Tet'ka Bracers (from setenals quest easy to obtain)
Chaos Guard
Visor of the flesh render
Tharens Goggles
Dusk and or Bloodstone
Various Clickys: Planiar Gird, Stoneskin, Haste, Invis, Jump, etc.
Various handwraps: stunning +10, Metaline, Paralizing, Destruction, Imp. Destruction, Various Elemental DMG, Greater Bane, tce.
Thats what I can think of off the top of my head

This is setup for a DPS Str Build, not a Dex/Wis AC Build.
If there is something a Monk needs that is not listed in this, please let me know.
**Greensteel Helm ~ +45 hp, +6 Dex Skills, Displacement (3/day), 20% Permanent Blur**
20% Perm Blur, 3x clickies of displacement, AND it all works with your ninja spy shadow fade.
**Shintao Cord ~ Constitution +6, Concentration +15**
**Kyosho's Ring ~ Wisdom +6, Exceptional Strength +1 (Holy Burst)**
Only for the set bonus of +2 to hit, and +2 damage.
**Litany Of The Dead ~ +1 All Stats, +1 To hit, ect ect ect**
It helps even out your stats, as well as helps you in combat.. Bloodstone MIGHT??? be bette?
**Epic Cape of the Roc ~ feather Falling, Except Strength +1 & +2 (Empty Yellow)**
No, you don't need the feather falling, however that +3 Strength you get is HUGE
(only slot a monk can get +2 Exceptional Strength without wasting a ring.)
**Epic Belt of the Mroranon ~ Strength +7, Heavy Fortification, Toughness (Empty Yellow)**
No reason to make a Min II Greensteel item if you can grind this out.
**Epic Spectral Gloves ~ Ethereal, Dexterity +7, Attack Bonus +4 (Empty Yellow)**
Lets face it, +4 to hit, when using Power Attack, is big.
**Calitomes' Ring ~ Charisma +6, Exceptional Dexterity +1, Shocking Burst**
This fits in your +1 Exceptional Dex, as well as adds +3 to your UMD, and Shining Star finisher.
**Mentau's Goggles ~ Necromancy Focus, Seeker +4**
You should have a seeker item, this give you +4 to confirm crits, and +8 to critical damage (on a x2)
**Epic Epic Red Dragonscale Docent/Robe (Empty Blue & Colorless)**
This gives you the ability to insert Good Luck +2 into your armor
SUPERIOR Fire Resistance (no more begging for it)
Eternal Fire = Flaming Burst that stacks with other flaming effects.
Bracers:
**Epic Bracers of the Hunter ~ Sneak Attack +5, (Empty Yellow)**
Gives you your sneak attack damage.
Exceptional Wisdom +1 in this slot so swapping bracers isn't a pain.
**Epic Jidz-Tet'ka ~ 25% Healing Amp, Exceptional Wisdom +1, Blindness Immunity (Empty Colorless)**
25% Healing Amp is a big help, when your tanking, otherwise, go with your SA damage.
Boots:
**Madstone Boots**
What else you gonna wear? You run fast enough as it is...
**Boots of Anchoring**
Of course, when you're running ToD.
**30% Striding Boots**
I'd suggest the Striding Boots from the Titan Raid Personally...
**Gyroscopic Boots of Striding - Balance +10, Striding +30%**
This SHOULD put your balance over 40.
Now as far as what to put into those Epic Slots you got laying around...
Good Luck +2 *Blue Slot ~ ARMOR*
+1 Except Con *Colorless Slot ~ ARMOR*
+1 Except Wis *Yellow Slot ~ BRACERS*
Greater False Life *Yellow Slot ~ BELT*
Resistance +4 *Yellow Slot ~ GLOVES*
Fear Immunity *Yellow Slot ~ CLOAK*
Protection +4 *Yellow Slot ~ CLOAK* If you plan on running greater heroism 100% of the time.
Protection +4 or Protection +5 for this gear layout, isn't needed.. the HIGHEST ac you could hope for, would be 70-73... and thats completely 100% buffed... honestly, it aint squat...
Stack displacement, blur and shadow fade for your AC.
20% 50% miss chances per hit on all mobs except ones with True Seeing.
25% Shadow Fade miss chance vs all mobs except ones that can hit incorps.

I'm a semi-new player, and I'm trying to prepare in advance for when I hit 20 and start raiding as much as my playtime allows..so as such I've compiled a list of gear that I want to aim for, and I'm wondering if my choices should be changed. My focus is going to be on DPS. I prefer the guard augments on items over heavy AC since well..I'm a DPSer, not a tank.
I'm currently a 32pt Dex based Halfling Dark Monk.
Here's the gear I chose to aim for:
Devout Handwraps (among other goodies)
Minos Legens (currently have)
Oremi's Necklace
Litany of the Dead
Tharne's Goggles
Dragontouched Vestments: Resist +5, Corrosive Salt, Disintegration (working on the right augments atm)
Epic Jidz-Tet'ka Bracers (have regulars)
Spectral Gloves
Barb STR Ring w/ Shocking Burst
Shintao WIS Ring w/ Holy Burst
Health Belt of Greater False Life (6 CON) -- (Currently have 6 con on neck..looking to switch it to this slot)
Greensteel Weave Cloak: Lesser Regen, +2 Dex Skills, Radiance I, Greater Fire Guard, Greater Good Guard, Radiance II
Greensteel Weave Boots: 10 HP +1 Con skills, 15 HP +2 Con Skills, Earth Guard I, 20 HP +3 Con Skills, Earth Guard II
This gives me: Corrosive Salt, Disintegration, Radiance 2, Earth Grab, Greater Fire Guard, Greater Good Guard..anything hits me it'll wish it didn't..
Any suggestions on changes? Keep in mind my aim is to keep the stat slots as is..ie; str/wis on ring, con on belt, dex on gloves.

It depends..
Fire Stance IV = 110 KI from hits, 60 or so Ki from crits per minute..
Add Crane to that, and you wont need any more Ki for your attacks...
Oremi's =
110 (give or take) Ki per minute. (1/2 what is needed to keep ToD going)
ToD requires 200 Ki per minute to keep on cool-down.
+ you'll want to be using Earth 3, Earth 4, FoI, EEE Finisher, FoD, Fire4 or Wind4, or Water4, whatever ya got.
those 6 + ToD will keep your ki strikes on cool down 100% of the time.
Now the amount of Ki needed for those, SHOULD be provided for you from Fire 4 stance (+1 per hit, +5 per crit), Capstone (+1 regen), AND from Crane if you take it...
You can always swap necklaces' if you find that the Shintao set isn't keeping up enough for you, however, in a situation where you need max base dps, i'd be wearing shintao's. (a necklace is easy to get, the ring isnt)

Well that would be true if you run crane. For instance on a dark monk with either Tortoise or Monkey and ormi's is the better way. I personally dislike Crane. I dislike the Negative to fort saves for just more Ki on crits which is 10% of the time. Instead I prefer Ormis set for +d6 fire damage and Ki on each hit keeping me in the Ki for all strikes including ToD Earth IV strike, etc.
It really falls to a personal preference. I say try them both. But Butlerfamilwa is right that the necklaces are much easier to acquire than the rings. I had all 2 necklaces long before I had any of the monk rings. but with enough time and patience you can pick up both sets and see what works for you.

I am honestly not a fan of the Shintao Set. While usefull for every other meele, it is more the "lazy monk's" dps increase.
Oremies with max fire and earth stance will produce better dps as you will be able to spam more earth strike IVs and IIIs (which do +16 and +12 damage as well as burst). I recently switch my halfling from Dex based wind stance to strength based fire stance with this set up, and he kills stunned mobs ULTRA fast now. Of course having a +4 con tome helped with getting earth stance 4!:D
As for the rest of the gearing I believe a seperate tab for Dex based AC builds needs to be made vs. strength based builds. The biggest gear difference being Dragontouched. So here we go=
1. Icy rainment is junk even for AC builds, DT with +4 insight wins (we assume unarmed for all monks)
2. Best Dex based DT= +5 resist, +5 protection, insight 4
3. Best Str based DT (for trash mobs)= +5 resist, Healing amp 20% (or magma guard for debuff), Radiance guard
4. Best Str based DT (for bosses)= +5 resist, Healing amp 20%, Earthen Guard (stoneskin proc will give DR against epic bosses that bypass DR epic)

It depends..

AC based DT armor is a waste if you're not fully going all out on AC everywhere else..which I'm not. And that's why I chose guards instead.

1. for a warforged str based monk why not the docent of quickening? 10% alacrity not worth it?
2. also, the sentinal bracers? what does the monk improved elemental stances really do?

Docent of quickening does not stack with haste, so it's not a must have item.
The improved stance buffs fromt he bracers are:
Fire stance-25% healing amp
Air stance-+? (forget bonus) insight bonus to jump
Earth- Ups your fist damage die one higher, so for a level 20 non tr monk fists do 2d12 dmg
Water-Low dc poison applied on attack on natural 20 (who thought this was a good idea?)

These dropped a couple days ago. I had Glacial Gloves... I offered to trade. The person with Tharnes accidentally gave them to the wrong person. They're now rotting in a chest.

First I want to say with all the gear and epic gear out there... The combination of gear is rather endless. I often find I am reconfiguring my gear as I gain other things. Unless you have a very generous guild it will take most that even attempt epic items a long time.
Goggles = Tharnes
Trinket = Bloodstone
Gloves = Gloves of Titan's Grip
Necklace = Scourge Choker -really great tanking or Monk Necklace or Bluefire Necklace
Boots = Firestorm greaves, Boots of Anchoring, Gyroscopic Boots
Cloak = GS HP cloak or Cloak of the Zephyr Cloak of Ice
Rings = Depending on your build, ToD rings a must, one holy and one shocking.
Belt = Belt of Thoughtful Remeberence, Electric Haze, Knost's Belt (IF WF Purging the
Pantheon is an Excellent choice.)
Armor = DT Armor and if WF I recommend the Quorforged Docent of Battle, the crafted version, not difficult to obtain.
Helm = Minos Legens or slot a GS item here.
Nifty Clicky items to get.
Planar Girds and/or Draconic Necklace (Greater Heroism)
Visor of the Flesh Renderer Guards (1/rest 7 Min Death Ward)
Mechanic Belt = 3/rest CL 15 Jump spell
Nyoko's Necklace = 3/rest CL 15 Invisibility Spell
DT Armor config ( I actually have 3 sets of DT with differing configs.)
Eldritch = 10% Healing Amp, Death Block or Heavy Fort, but if WF Greater Fire Resist
Tempest = Healing Amp 20% or Corrosive Salt or Crushing Wave Guard
Sovereign = Radiance or Disentegration Guard
I dont include resistance items in builds because I try to get two end reward abbott items that give you 15 minute nightshields, raises resistance and makes u immune to force spells.
AC I only moderately concern myself with, its not amzing and its not low but its good enough for most trash mobs and to work for epic ac defeats your DPS. So I build for moderate and incidental AC.

Nice cloak but you have to get it first, and if I had it, I would probably mentioned it, it is a nice cloak. IF your flagged, which can be a PITA sometimes, Then actually running abbott successfully (which is also dependent), Then getting the cloak to drop, then actually obtaining it. I didnt list Litany Either. I have to night shield clickies on my monk and still dont have a litany or wretched twilight, although I have seen 3 of each drop.
I pretty much tried to stay to things that reasonably obtainable or obtainable through the AH. Like Tharnes goggles, very nice raid item for melee's, VOD shortest route, no pre-flagging, no long pre-quest... Pretty much short run and jump right in. As opposed to abbott where the goggles room can be a PITA and/or Abbott gets ya with Inferno, etc...

Epic cloack of the Roc is the best monk cloak hands down now, since our tod rings will have burst efetcs insted of +2 exceptionals on them and the cloakc now has +1 and +2 exceptional str on it.
for the green steel the cloak is a good choice +45 hp and +10 con 1d6 elemental damage on a % strike rest is pretty much caster but for a +10 con and 45 hp went i took it.
ring sets for starters ring sets are good but to duel wield sets take a lot of slots till u up grade to epic then loosing the set bonus for epic ter will be worth it.

Epic cloack of the Roc is the best monk cloak hands down now, since our tod rings will have burst efetcs insted of +2 exceptionals on them and the cloakc now has +1 and +2 exceptional str on it.
Epic I dont mention due to the sheer fact you have to get the item, then have to get the shard and seal for it (maybe scroll will be on the AH) and then get enough raid tokens and then craft it. Min level 20 etc... It is a great item!, But like the wretched twilight easier said than done.
One of the reasons I laugh at some builds that have all thesee amazing items and unless you're super hard core and with a hard core guild, you would either need to be super lucky or take a considerable amount of time.

I think the reason alot of builds list rare/epic gear is because they are goals to shoot for. It's nice to have a long term plan for a character if it's one you intend to play that long.

Interested in using GS item here. Does the Balance of Land and Sky GS item that gives on-hit (not on-being-hit) Elemental damage work with Handwraps? Haven't met a monk who uses this. Figure it would boost my DPS.

Thats the trick, if you plan to play that long. Most players I seem to know seem to play a character up to 20 but still focus on their other toons and/or start working another up to 20. I guess I could understand day dreaming about the ultimate equipment.
Interested in using GS item here. Does the Balance of Land and Sky GS item that gives on-hit (not on-being-hit) Elemental damage work with Handwraps? Haven't met a monk who uses this. Figure it would boost my DPS.
If you are talking about the guards which proc on hit, yes they do because thats tied to you being hit not to your direct hits.

The wiki is wrong (not the first time). What you quoted does have a key point though: elemental mastery guard. It is a guard, which only goes off when you get hit. If people could make jewelry/clothing that gave them abilities when they attack that info would have been all over the forums. Every DPS build in the game would have that effect if it worked offensively instead of defensively.

The elemental mastery guard is the same effect regardless of which combination you made to get the effect. The picture doesn't even have the phrase "on being hit" but instead the much more clear "when attacked in melee".

Shouldn't Deathblock be on your list? Or are folks finding that they can get along without it? [On my high con sorc I've died a few times when I've forgotten to swap my haggle back to my silver flame necklace, so I tend to be be a bit paranoid about it.]

deathblock is nice to switch to when beholders are around, but other than that you'll have folks casting deathward when needed. You can also farm threnal for a deathward clicky.

I just hope there aren't anymore of these errors that could really waste hours upon hours of farming
