Is this a once per rest/x times per level, or can you use it as long as you have the necessary hp/sp? What's the cooldown?

You are only limited by your hp and sp. I don't know the cool down off hand.

I mash it all the time.

with a high crit range bodyfeeder (im currently using +2 bodyfeeder falchion of PG) its awesome.. u score crits frequently & the temp HP are used to power the next couple till you crit again :D
