At which levels do you recieve an additional Stat point?

I seem to recall getting one extra stat point at level 4, and maybe again at level 8? Will there be more, and if so, at which levels can I expect to see them? I am trying to plan my character builds a little better, and in advance this time.

4, 8, 12, 16, 20 iirc

Also, related to my original question, have you ever seen a Stat Tome higher than +2? That is the highest that I have seen in the AH so far.

+2 is the highest unbound, but you can get +3 and +4 as bound raidloot (well, from the jester of the festivult there is an unbound +4 tome too, though only 1 person got it this year...its just **** rare)
